Join The Night Ministry as a Case Manager – The Crib
Provide supportive, youth-centered case management in a safe and affirming overnight shelter.
About The Night Ministry
The Night Ministry is a Chicago-based nonprofit that provides human connection, housing, and health care to individuals struggling with housing stability or poverty. Our work is centered around two core service areas: Outreach & Healthcare and Youth Housing.
Through our Outreach & Health programs, we meet people where they are, on CTA platforms, in encampments, at libraries, and across city neighborhoods; offering consistent, compassionate care and vital health services.
Our Youth Housing programs provide shelter and supportive services to young adults, helping them build stability and work toward their goals in safe, affirming environments. With a history rooted in dignity, inclusion, and trust, The Night Ministry is committed to walking alongside those we serve with empathy and respect.
If you're passionate about creating meaningful change through relationship-building and practical support, we'd love to meet you.
What You'll Do
You'll work directly with young adults to assess needs, create individual case plans, and connect them with appropriate services. You'll also contribute to the overall program environment, offering group facilitation, supporting de-escalation, and helping maintain a consistent, trauma-informed space.
Key responsibilities include:
Providing case management and referrals for all residents of The Crib, with regular in-person engagement during overnight hours.
Developing individualized case plans and supporting youth in addressing both short- and long-term goals.
Maintaining up-to-date case notes and completing timely data entry in systems like Efforts to Outcomes (ETO) and HMIS.
Leading or supporting age-appropriate support groups and life skills workshops.
Managing an active caseload while staying aligned with all contractual and grant expectations.
Supporting conflict resolution and responding to crises in a trauma-informed, restorative manner.
Accompanying youth to off-site appointments and helping them navigate public systems and service referrals.
Participating in internal team debriefs, case reviews, and agency-wide trainings.
Representing The Crib during audits and compliance reviews in collaboration with program leadership.
