Treatment Navigator
- Salary Range: $20.04 - $24.48 based on relevant years of experience
- Location: Larimer county, Colorado
- Status: full time, 40 hours per week-Remote position with 1 day per week onsite.
Role Overview:
Responsible for guiding our clients through the healthcare process. This role will be responsible for educating clients on their healthcare options, and ensuring they have a good understanding of what their care process is going to look like.
Essential Duties:
- Provide exceptional customer service that is both professional and accommodating. Provide support to our clinicians and clients, both on and off site. Track clients through services and support attending providers with outreach and engagement efforts.
- Complete outreach calls to clients regarding services, answer questions regarding treatment options and requirements.
- Assist with adding referrals to the group list or the case management referrals list. Outreaching to clients for group enrollment or placement with a case manager for management services.
- Maintain electronic medical record charts. And assist with ongoing chart audits.
- Providing case management screening and linkage from referrals to be assigned to ongoing case management services. Completing an initial chart audit of case management chart compliance prior to assigning for ongoing case management services.
- Receive and screen telephone calls, prepare correspondence, route mail and perform other associated tasks of this nature.
- Sending clients’ immediate needs in referrals, treatment signature page and ROI’s for coordination of services as needed.
- Document all interactions in Electronic Health Record, at the front desk for certain letters and communication to be sent out to clients.
- Perform other duties as assigned.
Required:
- Bachelor’s degree
- At least one year of customer service experience required
- At least one year of healthcare related customer service experience preferred
- Prior data entry experience using MS Outlook, Word, Excel and an electronic medical record, preferred.
- Equivalent combination of education and experience may be evaluated and considered
