The Program Manager I of the Enhanced Care Management Program (ECMP) is responsible for the overall quality, management, oversight, and reporting of program goals and objectives.   This position is the direct liaison between the health plan(s) and NEVHC. Oversees team duties related to the ECM, insures interdisciplinary team is meeting regularly and that deliverables for ECMP in terms of patient outreach, engagement and enrollment are being conducted and documented accordingly.

Reports to: Chief Medical Officer and Director of Quality Health Equity and Innovation Consults with the Director Behavioral Health Services, the Director of Nursing, and the Chief Operations Officer for operational and programmatic issues

Qualifications:

1.    Must have a Bachelor’s degree with concentration in Social Work or Behavioral Health preferred, Nursing, Public Health or related health care field may also be acceptable. Master’s degree preferred.  

2.    At least two years of increasingly responsible experience in an administrative capacity.

3.    Demonstrated ability to understand care coordination of low income, chronically ill and patients with Substance use disorder (SUD) and Behavioral health (BH) issues. 

4.    Thorough knowledge of care coordination, and the principles of administration, supervision, public relations, and budgeting.  

5.    Ability to guide, direct and coordinate to meet program goals and objectives.

6.    Ability to speak and write effectively and to prepare clear, concise and complete reports.

7.    Ability to supervise and direct staff.

8.    Must be a good communicator, organized, multi¬ task and detail oriented, determined, self-motivated.

9.    Thorough working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures.

10.    Math skills enough to prepare routine spreadsheets, calculate/balance invoices/expense reports, and similar.

11.    Basic leadership skills.

12.    Demonstrated supervisory skills enough to supervise employees, students and volunteers.

13.    Computer skills in Microsoft Office programs (Word, Excel, etc.), and database systems.

14.    Effective verbal and written communication skills to communicate clearly and effectively with patients and others. 

15.    Very effective oral and written communications skills including public speaking experience.

16.    Effective work organization skills. 

17.    Ability to work effectively as a team player with a positive attitude.

18.    Ability to be flexible and work in a changing environment.

19.    Sensitivity to the different cultures represented among members and staff.

20.    Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.

21.    Demonstrated ability to set priorities for tasks to work effectively despite interruptions and under minimal supervision.   Self-starter, reliable and dependable.

22.    Fluency in English (speaks, read and write).

23.    Fluency in Spanish (speaks, translate, read and write).

24.    Ability to maintain absolute confidentiality about health care and another patient/client information.

25.    Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
