POSITION OVERVIEW:
The Case Manager in Permanent Supportive Housing is responsible for providing comprehensive support and advocacy for individuals experiencing homelessness or housing instability. This role involves collaborating with clients to create individualized service plans, connect them to necessary resources, and ensure they maintain stable housing. The case manager will work with families and single individuals located Alta Vista and Beechwood properties.
KEY RESPONSIBILITIES
Key Responsibilities:
1. Client Assessment:
- Gather information on clients’ housing history, health status, and social support systems.
2. Individualized Service Planning:
- Develop and implement personalized service plans that address housing stability, healthcare needs, employment, and social services.
- Set measurable goals and regularly review and adjust plans as necessary.
3. Support and Advocacy:
- Provide ongoing support to clients in navigating complex systems to access services such as healthcare, mental health, substance use treatment, and job training.
- Advocate on behalf of clients with service providers, landlords, and governmental agencies.
4. Resource Coordination:
- Identify community resources and programs that can enhance the clients' quality of life.
- Facilitate connections to case management services, support groups, and other relevant resources.
5. Monitoring and Follow-Up:
- Regularly check in with clients to monitor progress towards their goals and housing stability.
- Document all interactions, progress, and changes in client circumstances in compliance with program requirements.
6. Crisis Intervention:
- Provide immediate support and intervention during crises, including safety planning and
RELEVANT BACKGROUND
Experience and Education Qualifications
- Associate Degree preferred in Counseling, Human Services, Sociology or Social Work. May consider experience with at least 4 years in case management, crisis intervention or counseling.
- Must have experience and proficiency in database management and Microsoft Office applications.

Additional Requirements
- A valid and clean NYS driver’s license is required. Must have a reliable vehicle and be willing to transport program participants.
TERMS OF EMPLOYMENT
This is a grant position which may not exist beyond the duration of the grant period or any subsequent renewal of it. When the grant or any subsequent renewal ends, grant employees will be terminated and their positions will cease to exist. There is also no guarantee of continued employment for the duration of the grant.
A grant-funded employee may apply for and be considered for employment in another regular position in the same manner as any other applicant. If hired into a regular position, the employee’s salary from the grant position does not necessarily follow the employee into the new regular position.
WORK ENVIRONMENT
Usual human services office working conditions prevail. Must be willing to travel unaccompanied to areas where the population served lives. Must be available to work 37.5 hours per week and the work hours may include evenings or weekends.
