Alberta Health Services in partnership with Capital Care and Assisted Living Alberta is inviting applications for a Family Medicine Physician to join the Divisions of Continuing Care and Seniors Health in the Comprehensive Home Option of Integrated Care for the Elderly (CHOICE) located in Edmonton, Alberta, Canada.
The CHOICE program provides services for older adults who have complex health issues and want to remain living in their own homes. The goal of the program is to keep clients healthy, at home, and out of the hospital system.
CHOICE services may include:
· Medical monitoring and treatment by CHOICE physicians and nurses
· Medications dispensed by the CapitalCare pharmacy (via CHOICE)
· Physical and Occupational therapy
· Social Work
· Recreation (social activities)
· Personal Care (assistance with daily tasks as needed)
· Health and wellness education

Qualifications:
Successful Candidates will have a functional knowledge of resources and challenges in Alberta’s primary health care system. This knowledge will be supported by the candidate having completed a two year Family Medicine Residency and by ongoing community primary care experience.
We are particularly interested in candidates who have superior interpersonal skills with a demonstrated commitment to teamwork and peer relationships. The successful candidate should be committed to fulfilling clinical and non-clinical duties as a representative of Family Medicine.
The successful candidate must currently be certified with the College of Family Physicians of Canada and have medical licensure in the province of Alberta. This position is NOT eligible for AHS sponsorship of a College of Physicians and Surgeons Practice Readiness Assessment
