Community Health Worker

Position Overview

The Community Health Worker (CHW) will play a vital role in addressing health disparities and improving birth equity within underserved communities. Acting as a trusted liaison between healthcare providers and the community, the CHW will provide education, advocacy, and direct support to ensure that individuals receive the care and resources they need for better health outcomes.

Major Duties and Responsibilities

  • Health Education & Advocacy
  • Provide prenatal education, including nutrition, exercise, and the importance of regular doctor visits.
  • Assist families in understanding birth plans and available care options, such as midwifery and doulas.
  • Educate on risk factors like gestational diabetes, hypertension, and preterm labor.
  • Advocate for women’s rights in healthcare settings and address implicit bias.


  • Support Services
  • Offer guidance on navigating the healthcare system, including accessing insurance and clinics.
  • Provide mental health support and referrals for prenatal and postpartum depression.
  • Facilitate peer support groups to build community networks.


  • Improving Access to Care
  • Assist individuals in securing transportation, financial assistance, and culturally competent healthcare services.
  • Conduct home visits to check on high-risk pregnancies.
  • Help individuals navigate telehealth options for prenatal care.


  • Community Outreach & Engagement
  • Organize health fairs, screenings, and educational workshops on birth equity.
  • Develop culturally tailored educational materials.
  • Build partnerships with local hospitals, clinics, and social service agencies.


  • Data Collection & Monitoring
  • Track birth outcomes, assess community needs, and collect health-related data to inform policy advocacy.
  • Conduct surveys to identify barriers to care.


  • Policy & Advocacy
  • Support policy reform that expands healthcare access, Medicaid, and maternal health programs.
  • Advocate for gender-inclusive maternal healthcare.



Requirements

Qualifications for the Job

Education

  • Bachelor’s Degree in Public Health, Social Work, or a related field preferred. Equivalent experience may be considered.

Experience

  • Experience in community health work, case management, social services, or maternal health support preferred.
  • Experience working with diverse and underserved populations.

Key Competencies

  • Strong understanding of healthcare navigation and community resources.
  • Excellent communication and engagement skills.
  • Ability to collaborate with healthcare providers, community organizations, and social services.
  • Competence in Microsoft Office tools and data management systems.

Other Requirements

  • Must have reliable transportation with a valid driver’s license and insurance.
  • Ability to work evenings and weekends as needed.
  • Must be comfortable working in diverse community settings and outdoor environments.

Physical Requirements

  • Must be able to remain stationary for 50% of the time.
  • Ability to move within office spaces and community sites.
  • Occasionally lift up to 25 lbs. for event setup.
