Registered Nurse (RN) – Behavioral Health Care Facilitator

DPHC Pentagon, VA

 

 

Position Duties and Responsibilities:

  • Although the BHCF may assist the PCM and PCMH team in completing a number of nursing tasks, it is imperative that the BHCF is aware of their primary responsibility: to serve as a liaison between the patient and the PCM; and to facilitate care for patient’s PCM detailed treatment plan to include, but not limited to depression, anxiety disorders, posttraumatic stress disorder (PTSD) and pain.   
  • The BHCF ensures that at least 90% of their time is engaged in their primary work responsibilities.
  • Position will require attendance at a five day mandatory DHA BHCF training. Ability to complete training and meet core competencies must be demonstrated within three months of hire. The three-month threshold for demonstration of core competency skills will be extended if the DHA is unable to provide training opportunities within three months.
  • Facilitates patient care to achieve optimal health outcomes, quality and cost effectiveness.  Assesses, coordinates, and delivers care for adult primary care patients. Provides patient and family education according to credentialed provider’s instruction and documents patient and family understanding of credentialed providers care plan.
  • Works independently in the absence of direct oversight. Effectively uses appropriate communication format in addressing and resolving professional issues.  
  • Ensures compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment facility. Provides care within ethical and legal boundaries. Participates in peer review and monitoring of professional nursing practice with the ability to provide and receive feedback regarding advancement of professional practice and safe patient care.
  • Effectively communicates and collaborates with a diverse group for the purpose of informing and coordinating care to the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.  
  • Excellent written and oral communication skills to perform accurate documentation, both written and electronic, of all activity to include patient care hand-offs and care coordination in accordance with requirements. 
  • Recognize patients at risk for harm to self and/or others or experiencing adverse signs and symptoms and react quickly in emergency situations. Ability to calmly assess life-threating situations and appropriately coordinate/expedite life-saving intervention.        Competence in all required emergency procedures for the population cared for. 
  • Completes orientation and competency verification programs in accordance with (IAW) unit and hospital guidelines. 
  • Perform patient care and duties with minimal to no supervisory direction according to appropriate/relevant Nursing Care Standard and Nursing Code of Ethics.
  • Serves and participates in committees, functions, and other meetings as directed.  Provides relevant and timely information to these groups, and assists with decision-making and process improvement. Participates in customer service initiatives, performance and quality improvement measures and activities designed to enhance health services.
  • Assists behavioral health consultants (BHC) with educational classes as needed. Assist the BHC with administrative duties, for example, but not limited to scheduling as desired by the BHC.
  • Ensures a safe work environment, employee safe work habits and patient safety IAW regulatory agencies, infection control policies, and process improvement initiatives. Promote and contribute quality performance, performance improvement programs, and nursing practice in a setting that supports professional practice and sets a positive example; identify and deliver excellence in the delivery of nursing services and care to patients. Proactively identifies process issues that could lead to negative patient outcomes and participates in the appropriate safety reporting processes for the facility.
  • Completes all job specific training requirements on time.
  • Participates in local and DHA mandated activities as required and relative to program operations and reporting requirements.
  • Must maintain a level of productivity and quality consistent with: complexity of the assignment; facility policies and guidelines; established principles, ethics, standards, and scope of practice for professional mental health nursing as outlined by the American Nurses Association and the American Psychiatric Nurses Association; the American Hospital Association’s Bill of Rights for Patients, the Case Management Society of America (CMSA), American Accreditation Healthcare Commission/Utilization Review Accreditation Commission (URAC); Accreditation Association of Ambulatory Health Care (AAAHC);and other applicable DHA guidance and policies. Must also comply with the Equal Employment Opportunity (EEO) Program, infection control and safety policies and procedures.

 

Minimum Qualifications: 

  • Baccalaureate's degree in nursing. Graduate from an academic program, within the nursing specialty field of study that is accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
  • Certified/certification eligible by the American Nurses Credentialing Center (ANCC) as a Psychiatric and Mental health Nurse with 2 years full time experience in mental health (is preferred).
  • Must have at least 36 months of total nursing experience in a direct patient care clinical setting. 
  • Certified/certification eligible by the American Nurses Credentialing Center (ANCC) as a Psychiatric and Mental health Nurse with 2 years full time experience in mental health (is preferred).
  • A minimum of three years of experience as a registered nurse in an ambulatory or inpatient setting with the ability to perform nursing assignments of moderate to severe difficulty and experience with acutely and chronically ill adults. Ability to anticipate and articulate to health management and/or interdisciplinary team the patient’s needs, requirements, abilities, and limitations. 
  • A minimum of three years of experience with a variety of pharmacological agents used in patient treatment, including knowledge of the desired effects, side effects and complications of their use, as well as the accurate administration of the pharmacologic agent, particularly psychopharmacological agents.  Must possess a working knowledge of the administrative requirements for proper documentation of patient conditions including disease progress, patient acknowledgement of understanding recommended course of treatment, self-care, and follow up care. 
  • Current, full, active, and unrestricted license as a Registered Nurse.
  • Must be a US Citizen.
