General Summary:
Under direction of the Program Manager and with independent professional responsibility, the Case Manager II performs case management with individual program participants in their corresponding program; functions at a highly skilled level in such areas as assessment, individual goal planning, group facilitation, community resource development, file monitoring and review; reviews agency reports and develops summaries. The ideal candidate will have a strong understanding of the “trauma informed care” model, the ability to maintain a strength-based perspective and to work with homeless, chronically homeless and/or disabled individuals from diverse social and ethnic backgrounds. This position is committed to improving the quality of services provided by seeking input from participants, employees and stakeholders to ensure service delivery and accessibility is equitable to all persons served.
The Case Manager II is a full time, non-exempt position eligible for Medical, Dental, Vision, Life Insurance; 403 (b) retirement plan; paid holidays, and Paid Time Off. This position is employed by Goodwill Industries® of Sacramento Valley and Northern Nevada, Inc., through Next Move Homeless Services, Inc.
PRINCIPAL DUTIES & RESPONSIBILITIES:
Essential Functions:
- Screens clients either in person or by telephone to determine their needs and eligibility for program assistance.
- Performs overall assessment of program participant’s needs and develops individualized service plans with short and long term goals.
- Reviews case plans weekly to adjust goals where needed or appropriate.
- Educates program participants on issues surrounding living skills, parenting, financial capability, workforce development, health and hygiene, housing, and drug and alcohol abuse.
- Develops and maintains comprehensive lists of community resources and housing resources for use by program participants; refers program participants as appropriate.
- Establish individualized budgets with program participants and assist program participants in monitoring savings and expenditures and resolving any credit issues.
- Conducts visual inspections or units/rooms.
- Documents all contact with program participants; prepares all screenings, assessments, service plans, evaluations, and reporting records.
- Provides crisis intervention, referrals and collaborative consultation with any service providers working with the participant.
- Addresses unique financial circumstances or housing issues and focuses on ways of eliminating housing barriers to achieve a housing goal such as repairing credit, addressing a rental dispute, locating affordable housing, being informed of fair housing requirements of the Fair Housing Act, finding units accessible to our participants.
- Maintains and updates all participant files to ensure compliance.
- Acts as peer review committee and will conduct monthly file review of participant files.
- Transports program participants as needed.
- Attends and conducts meetings and trainings, as directed.
- Assists participants with mediation, critical thinking, and problem solving.
- Supports other Case Managers with participants and caseload when needed.
- Acts as point of contact and communication between Case Managers and Housing Specialists and Program Manager.
Other Duties/Responsibilities
- Establish and maintain positive relations with the general public, funding agency, participants and other staff.
- Ensures compliance with all Goodwill Industries Sacramento Valley Northern Nevada and Next Move policies, CARF standards, and safety and security regulations.
- Performs other duties as assigned.
Requirements
QUALIFICATIONS:
- Preferred Bachelor’s Degree in social services or related field and at least two years of successful experience in the delivery of human services or any combination of education, training, and experience, which would provide the knowledge and ability to complete the duties outlined above.
- Experience working with the homeless population and the barriers faced by the unhoused.
- Must have knowledge and experience with social casework objectives, principles and methods. Knowledge of Child Protective Services (CPS) and Adult Protective Services (APS).
- Must not present a direct threat to the safety and health of self, others or property.
- Ability to maintain a high degree of confidentiality and professional integrity.
- Ability to communicate effectively, both orally and in writing; define problems, collect data, establish facts, and draw conclusions; and effectively present information to management and/or public groups.
- Ability to demonstrate personal effectiveness through strong problem-solving and time- management skills.
- Ability to react under pressure, use good judgment in ambiguous situations, and be flexible/adaptable.
- Experience working with low-income adults and families from diverse social and ethnic backgrounds.
- Must be able to provide program and casework consultation to other staff members.
- Must have understanding of public welfare laws, principles of community organization, and effective use of public and private community resources.
- Must have experience assessing individual and group behaviors and incorporate knowledge of social and economic forces and its effects on individual and families.
- Must have experience and skill to establish and maintain effective individual and group relationships with participants, employees and community partners.
- Must have ability to facilitate different types of conversations with a diverse population
- No record or disclosure of criminal conviction that indicates a tendency toward theft, violence, dishonesty, deceit, drug manufacture or sale, moral turpitude, or predatory behavior.
- Must provide proof of identification and eligibility to work in the United States.
- Ability to demonstrate proficiency working with computers including but not limited to Microsoft Office Suite and database systems.
- Must pass a drug and background screening.
- Must have access to reliable transportation that does not interfere with performing the essential functions of the job description, cause tardiness, or unexcused absences.
- Must be able to perform the essential functions of the job description with or without reasonable accommodation.
- Possession of a valid California Driver’s License, Class C or higher, and proof of valid automobile insurance is required at the time of appointment. This position requires the ability to operate a motorized vehicle for the purposes of conducting agency business.
