The Orthopedic Nurse Navigator serves as a single point of contact to facilitate a smooth transition across all phases of care for patients undergoing total joint surgery, from the decision to proceed with surgery through the post-discharge phase of care.  This role is centered on enhancing the patient/family experience, coordinating resources, streamlining services and supporting excellence in surgical outcomes.  The Orthopedic Nurse Navigator works closely with the orthopedic practice managers, surgeons, surgical and inpatient nursing staff, home health agencies, skilled nursing facilities and community-based programs to effectively coordinate the plan of care prior to surgery and after discharge.  The position has a matrix reporting relationship to the Nurse Manager for PACU/Day Surgery/AM Admissions.

How You Will Fulfill Your Potential
Responsibilities

  • Interviews pre-surgical patients to conduct screenings, risk assessments and identify necessary referrals and resources to coordinate timely and comprehensive care to our patients undergoing total joint surgery. 
  • Provides support for the emotional and educational needs of the patient (family) preparing for total joint surgery, including:  clinical condition, treatment plan, preoperative and postoperative care procedures, access to necessary services (Prehab, Pre-admission Testing, diagnostic testing, Rehab therapists, etc.) potential complications and the patient’s role in recovery
  • Assists patients in understanding insurance coverage and advocates for unique needs of each patient to ensure available  benefits are utilized and patient’s needs are met
  • Serves as patient advocate and navigator to facilitate transitions through phases of care by providing support and education, utilizing  evidence-based practice guidelines, in collaboration with all members of the care team. 
  • Works closely with the interprofessional care team to develop and implement an individualized plan of care encompassing the patient’s physical, psychosocial and educational needs
     

Education/Accredited Programs

  • Baccalaureate degree in Nursing required
  • Minimum 2 years Medical/Surgical nursing experience, with orthopedic surgery experience required 
  • Prior experience in Care Coordination or Nurse Navigator role is preferred 
  • Advanced clinical decision-making skills; documented experience providing patient education and collaborating with other disciplines
  • Demonstrated leadership skills in professional nursing practice. 

Licenses/Certifications/Registrations

  • Current license and registration to practice Registered Professional Nursing in New York State
  • Specialty nursing certification preferred
     

Skills/Abilities

  • Advanced assessment and clinical decision-making skills
  • Strong collaboration and communication skills; able to articulate and adapt communication to varied educational levels and unique patient needs
  • Flexibility, creativity, leadership and organizational skills are essential
  • Ability to demonstrate sensitivity to individuals and families diverse care needs 
  • Ability to collaborate with physicians and various other levels of staff
  • Excellent verbal and written communication skills
  • Strong computer proficiency required, including Microsoft Office products, electronic medical record, databases and Internet
  • Demonstrated public speaking abilities.

Salary Range
The expected base rate for this Glens Falls, New York, United States-based position is $72,763.20 to $109,144.82xx per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
