Job Summary: The Resident Experience Director champions the wellness journey of Independent Living (IL) residents throughout the continuum of care. This key leadership role fosters relationships, addresses resident needs proactively, and integrates wellness services to ensure a high quality, person-centered experience. This position requires a clinical background (LPN or RN) and strong
commitment to enhancing resident satisfaction and well-being.

· Serve as a consistent and trusted point of contact for IL residents, following and supporting them through their continuum of care regardless of level, addressing questions, needs, or concerns as they arise.
· Complete IL assessments during the application process and conduct personalized check-ins during the first- and second-weeks post-admission, in collaboration with the Sales and Community Affairs teams.
· Schedule and conduct at least two wellness check-ins per year for each IL resident. Use insights from these visits to connect residents to wellness programming, Plymouth Care Connect, and Outpatient support services.
· Proactively support residents and families when a move to the next level of care is needed, coordinating closely with the clinical team and sales to ensure a smooth and compassionate transition.
· Identify, communicate, and coordinate with Plymouth Care Connect to ensure residents’ needs are met, and they remain safe, well cared for, and fulfilled.
· Partner with the current vaccination Lead to co-manage and eventually take over coordination of resident vaccination efforts, leveraging billing opportunities with Medicare after a 5-month transition period.
· Serve as a supportive team member during emergencies or IDPH inspections as needed, demonstrating flexibility and a calm, proactive approach.
· Maintain thorough records of wellness checks, admissions follow-ups, care transitions, and other relevant activities to ensure no resident or detail is overlooked.
· Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures.
· Maintain regular and punctual attendance at work and meetings.
· Attends in-service and education programs.

Requirements
JOB REQUIREMENTS:
· Bachelor’s degree in nursing, LPN or RN with license in the state of Illinois.
· 5 years of experience in geriatric Nursing and social services or a health-related service preferred; at a minimum.
· Five years’ experience in a CCRC and/or geriatric setting.

ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS: The physical activities of this position constantly require effective talking, hearing, sitting, walking, and standing functions. The position may require exerting up to 50 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.
OSHA Exposure Category: Category II – Position includes tasks that involve some exposure to blood, body fluids, tissues; possible could be required to perform Category I tasks.
INTELLECTUAL/EMOTIONAL REQUIREMENTS: The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing a variety of duties, often changing from one task to another without loss of efficiency or composure. This position requires the ability to deal with and find resolutions to problems and maintain a productive working relationship with employees and may be interrupted frequently to meet the needs and requests of others. It also requires adaptability to fluctuate between priorities daily while being responsive to needs as they arise.
