Are you passionate about supporting older adults and building strong community connections? EveryAge is seeking a compassionate and resourceful Service Coordinator to serve residents at Carolina Senior Living in Lexington, NC and Emmanuels Place in Statesville, NC—two Elderly Housing communities.

In this unique, combined role, you’ll be the go-to advocate and connector for residents, helping them access vital services and build supportive networks that enhance their quality of life.

Key Responsibilities:

• Provide general case management and referrals based on residents’ health, psychological, and social needs.
• Build and maintain strong relationships with local service providers to ensure flexible, affordable, and personalized support.
• Create and maintain a directory of community resources for residents and staff.
• Link residents to services such as transportation, meals, counseling, wellness screenings, and legal advocacy.
• Educate residents on available services, application processes, and their rights—offering advocacy when needed.
• Develop and monitor individualized case plans in collaboration with community partners.
• Establish volunteer support programs and foster informal support networks among residents, families, and friends.
• Offer training on tenancy responsibilities and aging-in-place strategies.
• Support staff with education on aging-related issues and service coordination best practices.

What We’re Looking For:

• A caring, proactive professional with experience in social work, human services, or a related field.
• Strong communication and organizational skills.
• A passion for working with older adults and building community partnerships.
• Ability to travel between Lexington and Statesville as needed.
