Summary of Position: The Patient and Family Navigator is responsible for incoming communications for all intake, referrals, and follow up for the Hospice program. This position will provide additional support to the admissions team, patients, and families. 

Shift: 8:00AM-6:30PM l 4 Days a Week

Education and Experience: 

  1. Graduation from an approved School of Practical Nursing.
  2. Current LPN licensure in the State of Ohio.
  3. Minimum of one year of experience in Medical/Surgical, Oncology or community health nursing.
  4. Hospice experience preferred.

 

Special Skills and Requirements: 

  1. Shall have a valid driver’s license.
  2. Excellent customer service skills.
  3. Detail orientated with excellent communication, documentation, and organizational skills.
  4. Ability to follow instructions.
  5. Ability to work independently without supervision.
  1. Travel to other sites as needed. Travel may be in inclement weather.
  2. Ability to lift 25 lbs. of supplies or equipment.
  3. Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
  4. Weekend and Holiday rotation

 

General Responsibilities of Position: 

  1. Answer referral calls and inquiry questions about hospice, hospice benefits, and palliative care.
  2. Provide consultative end-of-life care planning to patients and families aimed at matching prospect with the best service from our portfolio.
  3. Obtain orders from Medical Director and Attending Practitioners
  4. Work collaboratively with the Admissions team to facilitate the conversion of referrals.
  5. Assist with admission paperwork, including physician orders and data entry into the EMR.
  6. Assist the Admissions team with pre-admission planning, including ordering medication, DME, and set- ting up transportation for patients.
  7. Coordinate the admission with the discharge planner at the hospital when applicable.
  8. Coordinate with the finance department on verifying benefit periods.
  9. Organize all obtained information and enter the required data into the EMR.
  10. Schedule the admission nurse and coordinate with team managers.
  11. Update supervisor on all referrals, admission, and other Agency issues and needs.
  12. Update the admissions dashboard.
  13. Complete all documentation as required by the organization in compliance with Agency policy, accredi- tation bodies, and governing requirements.
  14. Prepare and complete pre-admission paperwork according to Agency requirements
  1. Audit new admissions for accuracy.
  2. Follow up with patients in the In Touch Program
  3. Assist with pre-admission activities, including but not limited to referrals, ordering medications, ordering DME, and setting up transport when needed.
  4. Complete referrals and pre-admit contact
  5. Obtain patient health information and enter required data into the EMR.
  6. Assist with data entry, including but not limited to physician orders, frequency orders, medication, and DME.
  7. Assist with QAPI audits
  8. Oversee admission binder process.
  9. Other duties as assigned.
