What you will do:
The Hospice Clinical Manager plays an exciting role in coordinating and supervising the plan of care for hospice patients, to include home, nursing home, assisted living and in-patient care. They serve as a liaison between the Hospice IDG team, patients, caregivers, vendors, facilities, and other providers. The Clinical Manager will oversee and monitor patient care based on provider orders, compliance, department policies and procedures, state and federal laws and regulations, ACHC, and best practice. They will inspire and support our hospice registered nurses and certified nursing aides, by setting clear expectations and fostering an empowered, engaged team. The Clinical Manager conducts supervisory visits and engages in staff development and collaboration efforts. This position takes administrative on-call duties, as well as participates in holiday staffing.
 
Required Education:
Bachelor’s Degree in nursing

Required Skills:
Nursing skills as defined as generally accepted standards of practice. Effective professional communication skills. Ability to multi-task. Ability to utilize Microsoft Office effectively.

Preferred Skills:
Experience with Home Care Home Base, Meditech

 
Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; CPR certified; current Texas Driver’s license; current auto insurance. Must have reliable transportation.
 
Preferred Licensure/Registration/Certification:
Hospice and Palliative Care certified; Wound Care Certified

 
Required Experience:
Three years experience in respective discipline, three years’ experience in supervisory position in healthcare administration.
 
Preferred Experience:
Hospice or Home Care experience
