The Patient Care Manager (PCM) oversees and coordinates all aspects of patient care services in a home health setting. This leadership role ensures the delivery of high-quality, compassionate, and compliant care for patients and families. The PCM supervises the interdisciplinary team, facilitates care planning, ensures adherence to regulations, and fosters a supportive environment for staff and patients.

Key Responsibilities:

  • Supervision & Management
    • Directly supervise clinical staff including RNs, LPNs, CNAs, and other care team members.
    • Conduct regular team meetings to review patient care and staff performance.
    • Provide coaching, mentoring, and performance evaluations.

  • Patient Care Oversight
    • Monitor patient care to ensure adherence to individualized plans of care.
    • Collaborate with physicians and other disciplines to ensure comprehensive care delivery.
    • Participate in and lead Interdisciplinary Group (IDG) meetings.
    • Assist in the field with patient care when needed.

  • Compliance & Quality Assurance
    • Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies.
    • Participate in quality improvement initiatives and audits.
    • Maintain accurate and timely documentation in the electronic medical record (EMR).

  • Admissions & Case Management
    • Support the admission process by reviewing referrals and assigning appropriate staff.
    • Oversee case management to maintain continuity of care and appropriate caseloads.
    • Coordinates effectively with liaison team to assure timely initiation of care.

  • Education & Training
    • Provide ongoing education to staff on clinical best practices, policies, and hospice philosophy.
    • Identify training needs and assist in the development of educational programs.

Qualifications:

  • Registered Nurse (RN) licensed in Arizona. California to be obtained once hired.
  • Minimum of 3-5 years of clinical experience, with at least 1 year in hospice or palliative care
  • 1+ year of leadership or supervisory experience preferred.
  • Knowledge of hospice regulations and clinical standards of care.
  • Strong interpersonal, organizational, and leadership skills
  • Proficiency with EMR systems and Microsoft Office Suite

Working Conditions:

  • Office-based with intermittent travel to patient homes, facilities, and team meetings
  • Occasional evening or weekend work may be required.
  • Must have reliable transportation and valid driver’s license.
