Are you organized, people-focused, and passionate about helping patients get the care they need? Join our team as an Intake Coordinator and play a key role in guiding patients from hospital to home. You'll be the first point of contact for patients, families, and referral partners — coordinating care, verifying coverage, and setting the foundation for an excellent home health experience.

Please note that this is a Non-Exempt role at $22 to $25/hr.

Responsibilities

  • Evaluate referred patients and gather required documentation, including history and physicals and Face-to-Face verifications
  • Verify insurance coverage and obtain prior authorizations for home health services
  • Coordinate with hospital discharge planners, case managers, and clinical teams to support safe, timely transitions home
  • Interview patients and families to understand home needs and build a clear picture for the care team
  • Complete and maintain accurate referral records and intake documentation
  • Serve as a liaison between our organization, hospitals, and insurance providers
  • Track referral data, attend team meetings, and keep leadership informed of any delays in care
  • Perform other duties as required for patient services

Requirements

  • At least 18 years of age
  • High school diploma required; prior experience in medical administrative support preferred
  • Strong written and verbal communication skills, including a professional phone presence
  • Comfort with computers, data entry, and standard office software
  • A friendly, professional demeanor and a talent for building strong relationships with patients, families, and referral partners
  • Strong organization and the ability to juggle multiple priorities with ease
  • Reliable personal vehicle and valid driver's license
