Position Summary:  

We are seeking a compassionate and skilled Mobile Doctor or Transitional Care Mid-Level Health Care Professional (Nurse Practitioner, Physician Assistant, Doctor of Osteopathy, or Medical Doctor) to provide comprehensive in-home medical care to bed-bound patients or those unable to leave their homes. This role involves delivering physician-level services including medical visits, medication management, wound care, podiatry services, diagnostic orders, and coordination of home health or hospice care. All patient care activities must be thoroughly documented in the electronic medical record (EMR) with appropriate progress notes, medication records, procedures, and ICD-10 coding.

Key Responsibilities:  

• Perform physician-level (Level 3-5) home visits for bed-bound or homebound patients, assessing medical conditions in a timely and thorough manner  

• Initiate and adjust new and existing medication regimens based on patient needs and clinical findings  

• Provide wound care management including assessment, treatment, and documentation  

• Deliver basic podiatry care such as nail cutting and foot assessments to prevent complications  

• Order and interpret diagnostic tests as clinically indicated to refine diagnosis and guide treatment plans  

• Coordinate and order home health services, hospice care, or other community resources to support patient needs  

• Document all patient encounters in the EMR with comprehensive progress notes that include clinical findings, diagnoses, treatment plans, medication changes, and procedures performed  

• Use correct ICD-10 codes for diagnoses and procedures as per clinical guidelines to ensure accurate billing and record keeping  

• Collaborate with other healthcare providers, caregivers, and interdisciplinary teams to ensure continuity of care  

• Ensure compliance with all regulatory, legal, and organizational policies related to patient care and documentation  

Qualifications:  

• Licensed and Board-Certified/Eligible MD, DO, NP, or PA with experience in home-based or transitional care preferred  

• Strong clinical judgment and skills in managing complex chronic conditions in frail or immobile patients  

• Experience with medication management, wound care, podiatry services, and diagnostics ordering  

• Proficient with Electronic Medical Records (EMR) systems and accurate, timely documentation practices  

• Knowledgeable in ICD-10 coding for diagnoses and procedures  

• Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams  

• Ability to work independently and manage a mobile clinical practice efficiently  

Work Environment:  

• Primarily home visits in patients’ residences, requiring travel within the designated service area  

• May require use of personal or company vehicle for transportation  

• Flexible scheduling to accommodate patient needs  

Physical Requirements:  

• Ability to navigate home environments safely, including stairs, uneven surfaces, and confined spaces  

• Ability to perform physical examination and minor procedures such as wound care and nail trimming
