Responsibilities:
- Assist in the development, review, and revision of the plan of care.
- Provide nursing care per the physician’s plan of treatment, individualized plan of care, as authorized by the client’s payer source.
- Observe, record, and report any changes in the client's condition to appropriate personnel and/or physician.
- Deliver ongoing instruction and supervision to clients, families, and health team personnel in the plan of treatment.
- Monitor and document the client's condition and provided care services using electronic charting.
- Participate in education programs, quality assessment activities, nurse meetings, and other staff meetings as required.
Requirements:
- Active Licensed Practical Nurse or Registered Nurse License
- Strong problem-solving skills and ability to multi-task
- Excellent communication skills
- Positive and professional attitude
