Cheer Home & Health Care, Inc. is hiring a Physical Therapist in San Diego. The Physical Therapist responsibilities will include:
Responsibilities:
- Provide evaluations, treatments, and education per Plan of Care
- Perform initial and ongoing patient assessments
- Develop and update treatment plans with care team
- Notify physicians of patient condition changes
- Document care notes promptly (within 24 hours)
- Establish therapy goals and deliver appropriate interventions
- Ensure patient safety, infection control, and proper equipment use
- Train patients/caregivers on therapy and equipment use
- Assess home environment safety and recommend solutions
- Maintain confidentiality and professional conduct
Requirements
- Active California PT license
- 3 years of experience
- Industry Experience, preferred
- Upbeat personality
- Strong Computer Skills
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance
- Valid driver’s license and proof of insurance required
- Cell phone with Internet Access
- Proof of Negative TB results
