Koda Health is seeking an Advance Care Planning Patient Advocate to guide patients and their families through clear, values-based conversations about future medical decisions. This role supports adults living with serious or chronic illness through telephone and virtual visits, empathetic discussions, and ongoing follow-up to ensure their care preferences are understood and honored.

Key Responsibilities
- Lead meaningful conversations that help patients articulate their values, medical preferences, and goals for care
- Support patients longitudinally through scheduled check-ins, education, and documentation updates as their health needs evolve
- Coordinate visit logistics with the Patient Support team to maintain accurate scheduling, strong follow-through, and a reliable show-rate
- Maintain awareness of program metrics and contribute to the overall efficiency and quality of patient engagement
- Identify patients who may benefit from palliative care or hospice services and communicate these needs to the appropriate care partners
- Document all patient interactions with accuracy, timeliness, and clinical clarity
- Participate in ongoing efforts to refine workflow, improve communication, and strengthen the patient experience
Your first three months at Koda looks like:
30 Days: Build proficiency in Koda’s platform, documentation standards, and care approach
60 Days: Manage your own patient panel and conduct independent ACP visits and follow-up appointments
90 Days: Demonstrate consistent performance in longitudinal engagement, documentation, and scheduling coordination while contributing thoughtful feedback to improve processes
Requirements
- Professional background in fields such as social work, nursing, care management, patient advocacy, geriatrics, hospice, palliative care, or comparable roles involving complex conversations
- Experience guiding difficult or emotionally sensitive discussions, ideally related to medical decision-making, goals of care, or chronic illness management
- Familiarity with progressive medical conditions such as dementia, cancer, COPD, CHF, or ESRD, including their impact on function and decision-making
- Experience supporting patients or families across multiple encounters or phases of care is preferred.
- Ability to work Monday through Friday during regular business hours
- Ability to work Central Time Zone or Pacific Time Zone hours if necessary
