Job Summary:
The Community Program Coordinator will plan, implement, and oversee community-based programs and events that connect residents to Care Alliance Health Center services. This role bridges the organization and the community by assessing needs, developing programs, managing logistics, building partnerships, and ensuring program objectives are achieved. The coordinator will also track outcomes, supervise volunteers, and contribute to continuous improvement efforts, with a focus on enhancing community engagement and promoting health and wellness across culturally informed populations.

Key Responsibilities:


Program Development:

  • Collaborate with the Outreach & Engagement Manager to assess community needs and create programs addressing education, health, social determinants of health, and social support.
  • Develop program materials, curricula, and resources that support program objectives.

Program Coordination:

  • Schedule, plan, and organize program activities, including securing venues, managing logistics, and preparing materials.
  • Coordinate outreach events, workshops, health fairs, and educational initiatives.
  • Assists the clinical outreach team with health education and basic clinical screenings such as HIV testing, blood pressure screenings, and blood glucose screenings when needed.

Staff and Volunteer Supervision:

  • Recruit, train, and supervise volunteers who support program delivery.
  • Provide guidance and support to team members during events and activities.

Outreach and Communication:

  • Build and maintain relationships with community members, stakeholders, and partner organizations.
  • Promote programs through outreach, engagement, and communications strategies aligned with Care Alliance’s mission.
  • Communicate program updates and outcomes internally and  externally.

Evaluation and Reporting:

  • Support the Outreach & Engagement Manager in collecting data and tracking performance metrics.
  • Use data to inform program improvements and support reporting requirements, including grants and internal evaluations.

Qualifications and Skills:

  • Bachelor’s degree in public health, Social Work, Community Development, or a related field.
  • Minimum of 2–3 years of experience coordinating community programs, outreach initiatives, or public health projects.
  • CHW certification (or willingness to obtain within six months).
  • Strong verbal and written communication skills.
  • Experience coordinating events and activities.
  • Budgeting and financial management skills.
  • Ability to prioritize and manage multiple tasks efficiently.
  • Skills in community needs assessment, program development, and program evaluation.
  • Relationship-building, leadership, and team supervision abilities.
  • Experience working with culturally informed populations, including communities with low-income and individuals      experiencing homelessness.
  • Flexibility to work evenings, weekends, and community-based events as needed.
