As a Housing Liaison, your primary role is to engage individuals with disabilities in the community or persons in recovery and assist them in meeting their basic needs, including finding safe and affordable housing. You will actively assist clients in resolving housing crisis situations, serve as the internal expert on housing resources in your designated service area, and collaborate with local Continuums of Care to identify individuals experiencing unstable housing and establish a network for supportive and stable housing.

Key Responsibilities:
Practice Housing First, harm reduction, and low-barrier service models when working with individuals or households.
Attend and participate in Coordinated Entry/Case Conferencing meetings within the local Continuum of Care.
Reach out to households currently not engaged in CPR or CSTAR services that are experiencing a housing crisis or homelessness.
Refer and/or transport individuals to help establish them in services for needs including behavioral health, substance use, physical health, employment, etc.
Build and strengthen relationships with local landlords, providing education on community programs and referrals for their units.
Connect with referrals from Continuum of Care programs to assist individuals with housing stability.
Utilize SOAR training to assist individuals in applying for benefits.
Assist households in identifying when they are ready to move on to other permanent housing options.
Participate in state-wide Housing Liaison training and calls.
Complete timely data collection and entry as required by DMH.
Become knowledgeable about all resources available within the community.
Serve as a resource to case managers, clients, and agency administration regarding housing and local/state resources.
Assist eligible households in obtaining identification documents required for housing or employment.
Help with the moving on process/transferring to other services and meet with tenants to evaluate their readiness to move on to other permanent housing options.
Reengage active or former participants who have become disconnected with services or who are at risk of returning to homelessness.
Help households to identify and connect with natural resources to prevent or end homelessness.
Understand HUD Quality Standards inspections.
Education and/or Experience Qualifications:
A Bachelor’s in Social Work, Psychology, Sociology, or other relevant case manager experience in housing; OR,
Associate of Applied Science in Behavioral Health Support from an approved institution eligible to provide community support services; OR,
Qualified to provide Community Support through specified educational background and experience as defined in community-support qualifications.
