Key Responsibilities
- Perform initial and ongoing assessments to develop and update personalized care plans
- Deliver skilled nursing services and specialized procedures per physician orders
- Complete timely, accurate documentation including OASIS assessments
- Educate patients and families on safety, medications, and health conditions
- Communicate with physicians regarding changes in patient condition
- Supervise and support LPNs and HHAs, including conducting supervisory visits
- Coordinate care with the interdisciplinary team and participate in team conferences
- Ensure compliance with all clinical, regulatory, and documentation standards
Requirements
What We’re Looking For
- Active RN license in the state of New Mexico
- At least 1 year of clinical nursing experience (home health preferred)
- Proficiency in OASIS documentation and home health regulations
- Strong clinical judgment and attention to detail
- Ability to work independently with reliable transportation
- Current CPR certification
- Compassionate communicator and collaborative team player
- Valid driver’s license and auto insurance
