The Hospice Liaison Nurse is dedicated to working with all departments and entities within Centra to support the continuum of care. Job responsibilities will include collaboration with Hospice, Palliative Care, and English One program Medical Directors and patient support staff (Nurses, Social Workers, Nursing Assistants, Attending Physicians, Clinical Effective Staff, Pastoral Care, Volunteers, etc.) to assist in the development of the patient plan of care extending from a hospital stay to return home. Home may represent any place of residence. Job responsibilities will focus on identifying patient and family needs and setting goals to achieve best results and outcomes. Liaison will collaborate with Hospice Physicians for eligibility. The Hospice Liaison Nurse will also collaborate with community nursing facilities and physician practices to expand the continuum of care to the level of care needed to find best results for patients and families.
Responsibilities
Evaluates referrals for eligibility criteria and appropriateness for Hospice and Palliative Care programs according to conditions of participation by completing consultation visits
Collaborates with insurance provider for eligibility and payment for services
Obtains Agreement for Services and Consent for Services from patient and families
Obtains patient information necessary to facilitate admission, transfer or discharge throughout the continuum of care into the most appropriate programs necessary to achieve patient needs
Collaborates and educates with units within the hospital and physician practices on Hospice Services
Identifies and develops education, practice, knowledge and experience to train and educate Hospice staff, hospital staff, nursing facilities and physician practices in End of Life Care
Completes documentation within E-discharge, STAR, McKesson, Amplicare, and Hospice EMR
Assist Hospice and Palliative care teams in decisions surrounding patient care needs, insurance coverage, placement throughout the care continuum, and utilizes a holistic approach to meeting patient/family needs
Participates in the Hospice Quality Improvement initiatives
Advocates for the Hospice and Palliative Care programs through communication with physicians, hospital staff, committees, and the community
May perform other duties as assigned or requested and job specification can be modified or updated at any time
Qualifications
Required Education: Graduate from an accredited Nursing Program
Preferred Education: Bachelors of Science Degree in Nursing (BSN)
Required Experience: At least two (2) years of RN experience
Preferred Experience: At least two (2) years of hospice experience
Required Certifications and Licensures: Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card.
