Job Responsibilities:- Conduct comprehensive assessments involving patients and their family members or caregivers.
- Apply specialized screening tools to assess current and future needs related to memory disorders and cognitive impairments.
- Provide dementia care management for a panel of patients with dementia and their caregivers.
- Collaborate closely with neurologists, geriatricians, neuropsychologists, social workers, genetic counselors, and pharmacists to deliver comprehensive care.
- Participate in team meetings to discuss patient cases and develop coordinated care plans.
- Collaborate with patients' primary care physicians to create personalized care plans that support independence and address the outstanding needs of each patient.
- Monitor and modify care plans according to patient progress and feedback.
- Provide mentorship and educational resources to patients and their families/caregivers to help them navigate the challenges of memory disorders.
- Address questions and concerns, providing emotional support and reassurance.
- Coordinate and administer follow-up visits to assess patient progress and adjust care plans as required.
- Communicate effectively with the healthcare team to ensure continuity of care and patient safety.
- Maintain accurate and detailed records of patient assessments, care plans, and interactions in compliance with healthcare regulations.
- Stay informed about the most recent research and evidence-based approaches concerning memory disorders and dementia care.
- Attend relevant training and professional development sessions to improve skills and knowledge.
- Encourage and support patients in maintaining as much independence as possible through personalized interventions and recommendations.
Qualifications
Education:
- Master of Nursing Degree.
- Graduate of an accredited Nurse practitioner program approved by the California Board of Registered.
Experience:
- One (1) plus years’ experience as Nurse Practitioner highly preferred in memory/dementia disorders.
Licensure/Certifications Requirements:
- Current CA RN License.
- Nurse Practitioner and Furnishing License.
- National Certification or eligibility for certification as a Nurse Practitioner.
- Current DEA License.
- Current BLS (Life Support Certification) required.
