POSITION SUMMARY
This position is responsible for day-to-day operations of the Memory Care Unit. Expectations are
to coordinate and implement programs and services designed for residents with cognitive
impairment; meet or exceed standards of care; encourage and assist each Memory Care resident
to function at their highest level of ability; and by word and action reassure families and friends
that residents are safe secure and well cared for. This position reports to the Executive Director.
ESSENTIAL DUTIES AND RESPONSIBILITIES
In addition to all components of the above position summary the following “Essential Duties and
Responsibilities” are essential to successful performance in this position.
• Assist residents with activities of daily living such as bathing, dressing, grooming, dining,
continence.
• Assist in the move in, transfer, and move out of residents as directed.
• Use approved charting criteria and procedures to record pertinent information in resident
charts. Ensure resident records and charting are current and up to date.
• Schedule Personal Care Assistants and develop daily tasks and make assignments.
• Follow-up weekly on bath schedules and vital signs (blood pressure, weight, and
respiration, etc.).
• Act as a point of contact during shift for the communication with supervisors, doctors,
and families.
• Immediately report all incidents or accidents involving residents to the
Wellness/Executive Director and record all necessary information on the Resident
Incident and Accident Reporting form.
• Contribute to team effort by being flexible in work assignments; by furnishing support,
taking initiative, and by understanding how this position affects and compliments all
other community positions.
• Maintain current departmental policies, procedures, and licenses, following company,
Federal, State, and local requirements.
• Maintain stimulating activity programs that challenge residents mentally, physically, and
spiritually and that match individual preferences and capabilities. Partner with the
Activities Director to ensure resident engagement during activities.
• Maintain environments that encourage memories, promote feelings of well-being, and
stimulate the senses in both personal and communal areas of the community.
• Provide appropriate training to staff, such as behavior management, redirection, and
communication tools pertaining to resident care.
• Incorporate State Department of Social/community Services/public Welfare/licensure
requirements during in-service training and orientation.
• Supervise Memory Care staff, provide training set and adjust rates of pay and hours of
work, provide informal and formal performance feedback, recognize both appropriate and
inappropriate behaviors/performances through rewards and discipline, and apportion
work among subordinates.
• Participate in New Hire Orientation and other regular associate meetings or functions.
• Perform activities on schedule while maintaining regular attendance and punctuality
within specified tolerance.
• Correct environmental hazards in an appropriate and safe manner.
• Check resident files weekly for appropriate and necessary signatures.
• Perform all other duties as assigned.
SUPERVISORY RESPONSIBILITIES
The incumbent in this position will directly supervise the Personal Care Assistants in Memory
Care.

• Ability to interact with co-workers, residents, or vendors.
• Ability to work weekends, evenings or night work if needed to ensure shift coverage.
• Ability to attend all required in-service trainings.
• Be willing and able to work in and resident care position.
• Pursue educational development in Alzheimer’s disease and related disorders.
• Demonstrate willingness to help other staff whenever needed.
• Report any suspected or observed abuse, neglect or misappropriation of property or funds
of resident to the appropriate licensing agencies.
• Operate department within budget.
EDUCATION/EXPERIENCE/CREDENTIALS
• A degree in gerontology, nursing, social work, or a related field is preferred.
• A minimum of 2 years of dementia experience in a supervisory role is required.
• 3-5 years of experience in assisted living and memory care is preferred.
• At least 21 years of age.
• Required Health Screening, TB Clearance, and Criminal Record Clearance.
• Any special licenses, certifications, and certificates (First Aid/ CPR Certified)
• Special skills needed to meet client needs [California Code of Regulations Sections:
85065(f)/87411(g)]
PHYSICAL REQUIRMENTS
• Physical ability to assist residents as needed.
• Able to lift, push, and pull as needed.
• Able to stand and walk for long periods of time.
• Able to bend, twist, and move easily.
