The Clinical Pharmacist in Oncology is responsible for optimizing medication therapy and ensuring safe, effective, and cost-conscious pharmaceutical care for oncology patients. This includes cancer therapy education, monitoring for treatment-related toxicities, managing supportive care, and addressing comorbid conditions. The pharmacist collaborates with healthcare providers to evaluate treatment appropriateness, monitor efficacy and safety, and enhance patient adherence. Pharmaceutical care services include assessing patient needs, incorporating age- and disease-specific characteristics into drug therapy planning, educating patients, adjusting therapy based on clinical response, and preventing or mitigating medication-related adverse events, in collaboration with ordering provider. Clinical Pharmacist Specialists also serve as departmental resources and act as liaisons across hospital departments, interdisciplinary teams, and external organizations.

Requirements:

  • Pharm D degree with a residency of one year or 3 years equivalent.
  • PGY2 in Oncology (preferred).
  • Current licensure from the Louisiana Board of Pharmacy.
  • Board Certification in Oncology (BCOP) preferred. If not certified, employees must complete ASHP Basics of Oncology in Pharmacy Certificate within 90 days and BCOP within 2 years of hire.

Responsibilities:

  • Provide safe, effective, and timely medication therapy.
  • Develop and maintain an active clinical pharmacy practice within oncology.
  • Participate in interdisciplinary rounds and outpatient clinic coverage (including weekends as assigned).
  • Collaborate with physicians, nurses, and other healthcare team members to optimize patient care.
  • Manage complex oncology medication regimens and supportive therapies.
  • Monitor treatment outcomes and adjust therapy as needed based on patient-specific factors and in collaboration with ordering provider.
  • Document all clinical interventions, assessments, and recommendations in the electronic health record (EHR).
  • Serve as a drug information resource for healthcare professionals.
  • Develop individualized pharmacotherapeutic plans and educate patients on their therapy.
  • Perform medication reconciliation and resolve medication-related problems.
  • Support pharmacokinetic evaluations and nutrition consultations as needed.
  • Assess patient-specific barriers to treatment, including financial or health literacy challenges.
  • Ensure compliance with institutional policies and applicable regulatory standards.
  • Precept pharmacy residents and students and provide ongoing education to staff.
  • Participate in and lead quality improvement and medication safety initiatives.
  • Serve in a leadership capacity by supporting clinical practice advancement in the profession of pharmacy.
  • Actively participates in multidisciplinary treatment conferences, sharing knowledge to improve patient outcomes.
  • Any other duties as assigned by Woman's Hospital.
