Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.
Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis.

JOB TITLE: Care Manager, Specialized Continuous Treatment Team
SCHEDULE: Monday - Friday 8:00am - 4:30pm
JOB SUMMARY: Care Manager, SCTT provides intensive care management services as part of an interdepartmental treatment team to Consumers ages 12 and older diagnosed with Intellectual and/or developmental disability and co-occurring behavioral health diagnosis who display severe dysfunction in daily living.
RESPONSIBILITIES:
Care Coordination:
- Assists with linkage, referral, and ongoing care coordination efforts with Consumers’ support team members to effectively address Consumer needs.
- Assists Consumers and families with accessing necessary mental health services, support groups, health related social needs, medical literacy needs, and resources available in the community.
- Provides intensive care coordination with MCRT, EPS, and hospital staff in the event Consumer experiences a crisis and/or is hospitalized. Participates in discharge planning and actively supports aftercare upon hospital discharge.
- Ensures smooth and timely transition of care to other services when needed.
- Transports Consumers to appointments when needed.
Care Planning & Documentation:
- Continually assesses Consumers’ strengths and needs via rapport building and assessment tools (DLA-IDD; CSSRS; PRAPARE) to collaboratively set SMART goals and support goal achievement.
- Documents timely individualized care plans, progress notes, and payor authorizations reflective of Consumer goals, progress, and needs.
Crisis Prevention & Intervention:
- Assists with crisis planning to proactively support Consumers in navigating potential crises safely.
- Responds to urgent/crisis Consumer needs with support from Supervisor, Crisis Services, and Treatment Team by developing plans and interventions.
Collaboration & Relationship Building:
- Engages in daily treatment team meeting, prepared to report on Consumer updates and collaborate effectively with team members.
- Builds and maintains strong professional relationships with Consumers and their supports, treatment team members, and community partners.
- Engages in professional development check-ins with Supervisor, prepared for case consultation and professional development topics.
Other Responsibilities:
- Responsible for attending all mandatory in-services, continuing education and annual health requirements.
- Comply with all Corporate Compliance policies, practices and procedures.
- Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director.
- Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator.
- Will be compliant with the relevant regulations concerning the privacy and security of consumer’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
- Participate in proactive team efforts to achieve departmental and company goals.
- Perform other duties as assigned.
- Follow all policies and procedures set by the Agency.
REQUIREMENTS:
Essential Skills and Experience:
- Bachelor’s degree in health-related field of counseling, psychology, social work, or other behavioral health field.
- Minimum 2 years of experience working with co-occurring (IDD/BH) population.
- Excellent communication, collaboration, initiative, organizational, and coordination skills
Essential Requirements for the Position:
