We are currently seeking dedicated On-Call Hospice Registered Nurses (RNs) to provide responsive, skilled care during overnight hours. This role is ideal for nurses who are passionate about making a meaningful impact during critical moments.

Role Summary

As an On-Call Hospice RN, you will be the primary clinical responder during overnight hours, addressing urgent patient needs, managing symptoms, and supporting families in their homes. You’ll work independently, using sound clinical judgment to assess situations and deliver timely, compassionate care.

Key Responsibilities

  • Respond promptly to after-hours patient needs, including symptom management, crisis intervention, and end-of-life support.
  • Conduct in-home visits to assess patient status and provide skilled nursing care as needed.
  • Collaborate with physicians and the interdisciplinary team to ensure continuity of care.
  • Educate and support caregivers in managing patient comfort and safety.
  • Document all clinical interactions accurately and in a timely manner.
  • Provide emotional support and guidance to families during critical moments.
  • Ensure appropriate follow-up and communication with day staff regarding overnight events.
  • Maintain readiness to travel throughout Contra Costa County during on-call hours.

Qualifications

  • Active California RN license.
  • Minimum 2 years of nursing experience; hospice or home health experience preferred.
  • Current CPR certification.
  • Valid driver’s license, reliable vehicle, and auto insurance.
  • Ability to work independently and remain calm under pressure.
  • Strong clinical assessment and decision-making skills.
  • Physical ability to assist patients with mobility and personal care needs.
  • Compassionate demeanor and excellent communication skills.
