SUMMARY
Maintain record of all medical appointments for each client. Ensure information regarding medical appointments is provided to Inter-Disciplinary Team (IDT)-including appointment outcomes and follow-up appointments. Ensure medical records for all appointments and emergent care visits are obtained and provided to the IDT.
ESSENTIAL FUNCTIONS
• Track all medical appointments for each client
• Inform IDT of results of all medical appointments
• Track upcoming medical appointments and keep IDT informed of medical appointment schedules
• Maintain contact and keep lines of communication open with medical providers in the community
• Obtain medical records for each medical appointment
• Obtain medical records for all emergent care visits
• Ensure new orders are on the computer and new MAR is printed
• Ensure clients have all required medications
• Check ISP schedule and attend ISP as assigned by the Health Services Director (HSD)
• Attend all Nursing staff meetings as scheduled
• Ensure implementation of vitals are documented in the computer
• Coordination of Individual Support Plan process as it relates to medical issues
• Coordinating information regarding doctor’s orders
• Assist in nursing orientation as assigned by the HSD
• Assist in the maintenance of medical records to ensure complete, accurate and pertinent information of all health conditions
• Notify HSD/RN Supervisor and physician of client needs
• Transcribe physician orders accurately and review orders of clients assigned at least weekly to ensure accuracy of transcription of orders and administration of medication
• Follow facility and pharmacy procedures for ordering and receiving medications
• Attend training and follow Behavior Intervention Plans
• Provide documentation of all medical and nursing care provided
• Fill-in as LPN on duty
• Administer medications and treatments as ordered
• Accompany and assist physicians with examinations as required
• Performs other duties and responsibilities as assigned.
QUALIFICATIONS
Education & Experience:
• Associates degree in Nursing or graduate of LPN program required
• Current TN LPN license
• CPR required
• Valid TN driver’s license required
Physical Requirements:
• Prolonged periods of sitting at a desk and working in front of a computer.
• Regularly required to verbally communicate, listen and give presentations as needed.
• Ability to lift repetitively a minimum of 50 pounds; tasks may include lifting, repositioning, turning, bending, squatting, etc.
