Job Summary: The Ambulatory Care Pharmacist provides direct, outpatient patient care by managing medications, educating patients on their health and drug therapies, and collaborating with physicians to optimize patient care plans. This role requires work in specialty pharmacy operations, 340B program understanding, and familiarity with both provider office and retail pharmacy workflows.
Essential Job Functions and Responsibilities:
- Provide direct patient care in collaboration with physicians and other healthcare professionals, optimizing pharmacotherapy outcomes in chronic disease states.
- Conduct medication therapy management (MTM), comprehensive medication reviews, and disease state education for ambulatory patients.
- Collaborate under a collaborative practice agreement with providers to establish, implement, and monitor evidence-based care pathways and treatment plans.
- Navigate and ensure compliance with 340B program requirements, including awareness of eligible entities, carve-outs, and billing implications.
- Coordinate with Bristol Hospital Pharmacy and/or specialty pharmacies for initiation, prior authorization, and monitoring of specialty medications across multiple therapeutic areas.
- Assist Bristol Hospital liaison with retail pharmacy and provider office workflow, ensuring smooth communication regarding refills, medication access, and patient adherence.
- Support clinical program development, quality initiatives, and process improvement activities to enhance patient care and operational efficiency.
- Maintain current knowledge of regulatory, payer, and accreditation standards relevant to ambulatory and specialty pharmacy practice
Qualifications
Minimum Requirements:
- Doctor of Pharmacy (PharmD) degree from an accredited College of Pharmacy or a Bachelor of Science degree in Pharmacy
- Active Connecticut pharmacist license
Preferred Education/Experience:
- PGY1 residency training or equivalent ambulatory care experience
- 2 years of ambulatory or specialty pharmacy experience across multiple therapeutic areas.
- Specialized certification/training (e.g., BCPS/BCACP, CDE, anticoagulation management, MTM, etc.)
Bristol Hospital Mandated Educational Requirements:
- General orientation at time of hire.
- Fire/Safety/Infection Control annually.
- Other programs as mandated by Bristol Hospital
Special Considerations:
- Working knowledge of 340B program structure and compliance requirements
- Strong understanding of both retail pharmacy workflows and clinic-based logistics, including EMR documentation, refill protocols, and prior authorization coordination
- Excellent communication, relationship-building, and patient engagement skills
- Ability to work collaboratively in a multidisciplinary environment and manage competing priorities efficiently
Physical Requirements: Minimal physical requirements. Long periods of sitting or standing may be required.
Work Environment: Office, Pharmacy and Patient Care locations. Limited exposure to patient elements and hazardous chemicals/pharmaceuticals.
Cognitive Requirements: Literate in English, verbal and written communication skills, ability to perform pharmaceutical mathematical calculations. Computer and keypunch knowledge. Critical thinking skills.
