As a wellness coordinator, you will be responsible to ensure the coordination of health services for individuals receiving wellness coordination. This will include face to face contacts with individuals, training for staff working with individuals, consultation with other providers, doctors, nurses, hospitals, and updating the Interdisciplinary Team on health updates.

Job Details:

Specific Job Activities:

  

· Medical Administration: Collaborate with the health office team to provide proper medical treatment for individuals served and maintain up-to-date and compliant medical records for all individuals served. 


· Medical Administration: Coordinate appointments with physicians and health office assistants, ensuring the routine health care needs of individuals are met. 


· Medical Administration: Implement agency policies and procedures related to contagious diseases.


· Training: Participate in the education and training of staff and individuals on health and safety matters. Serve as a consultant for all Program Areas.


· Training: Participate in training/certification of staff on Medication Administration and administering TB tests.


· Case Management: Participate in Individual Program Plan and team meetings to address medical needs and objectives for individuals as required. 


· Case Management: Assist in pre-admission evaluations, program design, and exit plans for individuals. 


· Case Management: Coordinate wellness initiatives for eligible individuals.


· Reporting and Record Keeping: Monitor state and federal regulations to maintain compliance.


· Health and Safety: Support administrators in maintaining agency policies related to health and safety. Promote agency-wide safety, housekeeping, and proactive preventive maintenance while delivering quality services. 


· Help develop and maintain annual wellness plan for those receiving wellness coordination.


· Upload quarterly wellness report into the portal for team review. 


· Participate in Nurse on Call rotation.

Requirements

1. Education and Experience: Must be a Licensed Practical Nurse and have preferred 1-year experience working in community settings utilizing community resources. 


2. Valid Driver's License


3. Must meet agency insurability requirements.


4. Physical Demands: Must be able to bend, stoop, or stand for long periods of time, and as needed assist in the lifting of individual served. Must be able to wear all required personal protective equipment such as, but not limited to: gloves, masks, gowns, face shields, shoes, ear plugs, etc. Must be able to follow all required safe and universal precaution practices during a pandemic and as required otherwise.
