Job Summary:

Join our Community Care Team as a Community Health Worker (CHW) to make a tangible impact on the health of complex, chronically ill residents in Howard County. You will connect individuals with essential resources, foster chronic disease management, and provide health education to help them achieve their wellness goals while minimizing preventable hospital visits. As a key member of our multidisciplinary team with Community Health Nurses and a Licensed Clinical Social Worker, you will create and monitor care plans, conduct regular home visits, and support clients through personalized interventions.

Key Responsibilities:

  • Build trusting relationships with clients and their families, offering support and encouragement.
  • Conduct regular follow-ups via phone, home visits, and community outreach.
  • Enhance clients' health knowledge and independence through education, counseling, and advocacy.
  • Perform intake assessments and facilitate referrals to primary care and community services.
  • Assist clients with scheduling and transportation for medical appointments.
  • Collaborate with healthcare providers to ensure coordinated care and report client progress.
  • Maintain comprehensive knowledge of community resources, including transportation and social supports.
  • Communicate consistently with the Community Health Nurse regarding client health status and barriers.
  • Document program activities accurately and timely.
  • Serve as a preceptor in HCGH's accredited CHW Training Program, providing mentorship to students.

Education & Experience:

  • Bachelor’s Degree preferred; Associate's Degree required, with a focus on behavioral, community, or public health.
  • Experience in community or home settings is a plus.
  • Proven ability to work in a multidisciplinary team and engage with diverse populations.

Licenses & Certifications:

  • CPR certification required within 60 days of hire.
  • Valid driver's license and reliable transportation necessary for fieldwork.

Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
