About the Psychiatry Program Coordinator Position

The Psychiatry Program Coordinator provides direct support to the behavioral health and psychiatry team by coordinating patient care, managing referrals, and ensuring smooth communication between patients, providers, and care teams. This role plays a key part in patient engagement and continuity of care by managing scheduling, follow-up, and transitions between services. The coordinator ensures timely access to behavioral health and psychiatric care and supports high-quality, patient-centered service delivery.

Duties include but are not limited to:

  • Serve as the primary point of contact for patients receiving psychiatry and behavioral health services, providing compassionate and professional support. 
  • Coordinate and schedule patient appointments for psychiatry and behavioral health providers. 
  • Monitor daily schedules and patient flow to ensure efficiency and accuracy. 
  • Perform outreach to patients who miss or cancel appointments to reschedule and identify any barriers to care. 
  • Manage all behavioral health and psychiatry referrals, ensuring documentation, scheduling, and communication are completed in a timely and accurate manner. 
  • Track referral status and communicate updates to providers and care team members. 
  • Facilitate care coordination following hospitalizations, including scheduling follow-up appointments, and obtaining hospital discharge records and summaries. 
  • Collaborate closely with primary care providers, behavioral health consultants, and care managers to support integrated care workflows. 
  • Maintain detailed and accurate documentation in the electronic health record (EHR) for all scheduling, outreach, and coordination activities. 
  • Support patients in connection with additional community resources as recommended by the care team. 
  • Participate in team meetings, case reviews, and quality improvement activities related to behavioral health operations. 
  • Assist with grant reporting. 
  • Ensure all activities comply with HIPAA, confidentiality, and organizational standards.

Requirements:

Qualifications, Education, and Knowledge

  • High school diploma or equivalent required.

Associate or bachelor’s degree in healthcare, behavioral health, psychology, social work, or related field preferred.

  • Minimum of two years of experience in healthcare, behavioral health, or care coordination support role preferred.
  • Experience working in outpatient behavioral health or integrated primary care setting strongly desired. 
  • Knowledge of behavioral health services, referral management, and community mental health resources preferred.
  • Attendance required at assigned CHDC site(s)

Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.
