Position Summary

The Placement Manager will work with the Program Director to ensure that continued enrollment at maximum capacity. The Placement Manager works with professional staff to ensure that all areas of need within each transitioning child’s ecology are identified to assure coordinated placement of students within the program.  The Placement Manager will also ensure continued communication with external agencies, families, and assure a congruent system of communication in placement procedures.  The Placement Manager is responsible for coordinating and implementing transitional activities specific to reintegration into a less restrictive environment for students, families and school districts.   

Essential Responsibilities

  1. Coordinate with students and classroom team to conduct initial transition/employment/career assessments. Meet annually to discuss and update goals and progress on transition plans.  
  2. Participate in team meetings in which IEP goals are identified for students who are nearing readiness for reintegration or beginning work program. 
  3. Provide classroom teams forms for reintegration progress documentation and collaborate on establishing goals and timelines. 
  4. Coordinate all meetings with school district/work employers, classroom teams, and Coordinators of Program Services for school tours and meetings.
  5. Conduct an initial pre-admissions/admissions interview with the family, guardian, student and/or appointed person to gather comprehensive background and current information. 
  6. Utilize technology through electronic documentation, electronic communication, and use of the internet to supplement programming.

Qualifications

  • Clearances. State Police; FBI clearance; child abuse clearance; CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training; any additional background checks/clearances required by state governing bodies.
  • Valid driver’s license and current vehicle insurance.
