Oklahoma City Indian Clinic (OKCIC) is a non profit corporation that provides culturally sensitive health care to the American Indian population. OKCIC services not only include basic medical care, but also dental, optometry, behavioral health services, fitness, nutrition and other family programs.

We are hiring a Dementia Care Navigator! The Dementia Care Navigator will play a key role in advancing culturally relevant, evidence-based outpatient care and supportive services for patients and families affected by dementia. This position supports the implementation of a comprehensive dementia care program funded through the Indian Health Service (IHS) Addressing Dementia in Indian Country (ALZ) grant.
The Dementia Care Navigator provides direct support to patients and caregivers, coordinates resources, collaborates with interdisciplinary teams, and engages with the community to improve awareness, access, and outcomes related to dementia care.
Job duties include, but are not limited to:
- Provide psychosocial assessments, counseling, and case management for patients across outpatient services, with emphasis on those impacted by dementia and their caregivers.
- Assist patients and families in navigating community and social service resources, including respite care, housing, financial assistance, mental health services, and caregiver supports.
- Document assessments, care plans, and patient interactions in accordance with organizational and regulatory standards.
- Facilitate caregiver support groups and educational workshops related to dementia care, while also participating in broader patient/family education activities.
- Provide crisis intervention and support for patients or caregivers experiencing distress.
- Serve as a liaison between patients, caregivers, the clinic, and external agencies, ensuring smooth coordination of services.
- Collaborate with interdisciplinary staff (e.g., physicians, nurses, pharmacists, behavioral health professionals) to ensure patient-centered care.
- Contribute to staff training, program evaluation, and performance improvement activities.
- Design, implement, and oversee the dementia care initiative in collaboration with the principal investigator, including policies, workflows, and care coordination models.
- Support the principal investigator in collecting and reporting program data, including outcomes tracking and quality improvement measures.
- Support dementia awareness and recognition efforts within the clinic and community, including collaboration on timely diagnosis and interdisciplinary assessment processes.
- Participate in culturally relevant outreach strategies to increase dementia awareness and resource utilization in the community.
- Assist with documenting and disseminating best practices in dementia care, and contribute to planning for program sustainability beyond the grant period.
- Support management in the execution of events that promote dementia and elder care, including coordination of logistics, communication, and patient/family engagement.

Qualifications
- Due to the safety-sensitive nature of this position, medical marijuana license holders who test positive for marijuana will not be qualified for this position.

- Bachelor’s or Master’s degree in any field
- Minimum of five (5) years of experience in a healthcare or social services setting
- At least 2 years in dementia care, geriatrics, or related field.
- Current Oklahoma social work license (LMSW or LCSW) OR ODMHSAS Case Management certification preferred; if not currently licensed or certified, must obtain within 60 days of hire.
- Current cardiopulmonary resuscitation (CPR) certificate.
- Advanced certifications (e.g., crisis intervention, grief counseling) preferred.
- Prior experience in program management, grant administration, or healthcare leadership strongly preferred.
- Demonstrated ability to work collaboratively in interdisciplinary teams and with Tribal/AI/AN communities.
- Strong understanding of culturally sensitive care and the historical experiences of AI/AN population.
- Excellent organizational, communication, and leadership skills.
