What You’ll Do:
- Evaluate and support residents and families with social, emotional, and psychosocial needs
- Provide guidance in areas such as conflict mediation, crisis management, abuse and neglect prevention, advance directives, and community resource connections
- Collaborate as part of an interdisciplinary team to promote engagement, understanding, and adaptation to care needs
- Participate in care planning and assist with Individualized Service Plans (ISPs)
- Maintain accurate, detailed documentation in electronic medical records
- Support new admissions and transitions by partnering with the Director of Community Relations
- Educate staff and families on psychosocial topics and resident well-being
- Uphold Heritage’s mission, vision, and core values in every interaction
What Makes a Social Worker Successful at Heritage:
- Compassionate and resident-focused approach
- Excellent communication and mediation skills
- Ability to work both independently and collaboratively
- Strong understanding of geriatric care and senior living settings
- Knowledge of Wisconsin state regulations for CBRF and/or RCAC
Qualifications:
- Bachelor’s degree in Social Work or related field required
- Licensed Social Worker (LSW or equivalent) in the State of Wisconsin preferred
- Experience in long-term care, assisted living, or senior services strongly preferred
