PRIMARY FUNCTION: The job of Teacher Hospital/Homebound was established for the purpose/s of providing the hospitalized or homebound student with an instructional program which will prepare him/her to re-enter school with minimum loss when he/she is physically able, or a special program to fit the need of the chronically or terminally ill student who may not be able to attend school.

Must hold or be eligible to obtain Georgia clear renewable teacher certification in ECE, 4-8 and/or 6-12 content fields, and must meet the Georgia definition of "highly qualified"; Valid Driver’s License |

ESSENTIAL DUTIES:
- Demonstrates prompt and regular attendance.
- Adapts classroom work for the purpose of providing students with instructional materials that address individualized learning plans within established lesson plans provided by the student’s teacher of record for each subject area.
- Administers required testing programs, subject specific assessments, etc. for the purpose of assessing student competency levels to assist the teacher of record in developing individual learning plans.
- Advises parents and/or legal guardians of student progress for the purpose of communicating expectations; student's achievements; developing methods for improvement and/or reinforcing classroom goals in the home environment.
- Assesses student progress towards objectives, expectations, and/or goals for the purpose of providing feedback to students, parents and administration.
- Collaborates with instructional staff, other school personnel, parents and a variety of community resources for the purpose of improving the overall quality of student outcomes.
- Counsels students for the purpose of improving performance, health status, problem solving techniques and a variety of personal issues.
- Implements academic, behavior, health and nutrition plans as provided by the teacher of record (e.g. literacy activities, stories, songs, math/science activities, art, etc.) for the purpose of ensuring compliance with regulatory requirements, established guidelines, and performance standards and encouraging parent involvement.
- Instructs students for the purpose of improving their success in academic, interpersonal and daily living skills through a defined course of study as provided by the teacher of record.
- Participates in a variety of meetings for the purpose of conveying and/or gathering information required to perform functions.
- Prepares a variety of written materials (e.g. attendance, anecdotal records, etc.) for the purpose of documenting student progress and meeting mandated requirements.
- Provides instruction for the student who has a medically diagnosed physical condition, which is non-communicable for the purpose of maintaining an educational setting to students that are restricted to their home or hospital.
- Responds to questions from a variety of sources (e.g. other teachers, parents, administrators, etc.) for the purpose of resolving issues, providing information and/or direction.
- Performs other duties as assigned by appropriate administrator.
