Are you a dynamic leader looking to make a difference in the lives of others? St. Joseph’s Center, with a history of 135 years of helping and supporting people in our community, is looking for a Program Coordinator to lead our Adult Day Services in Wilkes-Barre. In this role you will get to promote the safety and wellbeing of individuals with special needs while directing and guiding a team of staff in implementing individualized service plans. The primary responsibilities of the Program Coordinator is the development, implementation, and monitoring of the Individual Program Plan for up to 30 participants and the supervision of all the Adult Day Program staff.

What You'll Do
- Oversee the development, implementation, and monitoring of Individual Program Plans for up to 30 participants.
- Supervise and support Program Assistants, ensuring staff training aligns with participant health and safety needs.
- Ensure compliance with Department of Public Welfare 2380 regulations and St. Joseph’s Center policies.
- Coordinate and attend ISP meetings, and ensure timely completion of assessments and documentation.
- Maintain a daily schedule of meaningful activities aligned with each participant’s goals.
- Support participants with personal care and therapeutic needs as needed.
- Maintain accurate program records and collaborate with professionals, families, and external agencies.
- Facilitate staff training and ensure regulatory training requirements are met.
- Administer medications and maintain certifications as required (e.g., CPR Instructor, Certified Investigator).

What's Required of You:
- Bachelor’s Degree in a related field.
- Minimum of 2 years’ experience working with individuals with disabilities.
- Valid PA Driver’s License.
- Strong communication skills and a team-oriented mindset.
- Ability to meet physical demands of the role, including occasional lifting and mobility support.
