Extended Care Nurse - Center Township at Gateway Rehabilitation Center Corp HQ

Gateway Rehab Center (GRC) has an opening for an LPN or RN Extended Care Nurse in Center Township, PA. In this role, you will provide assessment, nursing diagnosis/treatment planning, and direct nursing care to GRC patients. This position receives supervision and instruction from the Executive Regional Director of Nursing. 

Responsibilities

• Conducts patient triage, assessments, reassessments, medical interventions, phlebotomy & POC testing as needed. 
• Complete audits for the extended care programs as required. 
• Interacts with the physician through coordination and the provision of assistance via in person, telephone, med wand or other platforms.
• Collaborates with pharmacy on medication issues. 
• Coordinates with all GRC departments as appropriate and care coordination with outside agencies as needed. 
• Attends GRC mandatory in-services and trainings.

Knowledge, Skills, and Abilities

• Excellent oral and written communication skills
• Strong computer skills.

Requirements

• Pennsylvania or Compact RN/LPN unrestricted license.  
• One year nursing experience, addiction medicine and med surg nursing experience preferred. 
• Active CPR certified required.

• PA Criminal Background Check
• Obtain a PA Child Abuse and FBI Fingerprinting Clearances
• Pass Drug Screen
• 2-Step TB Test  

Work Conditions 

• Working conditions include potential high-risk exposure to blood borne pathogens and low-risk exposure to respiratory infectious agents.
• Moderate physical demands.
• Significant mental demands. 
• Travel between various locations.
• Operation of various office and safety equipment.
• Use of various healthcare equipment and measurement devices.
• Requires mandatory staffing coverage when necessary to provide a safe treatment environment for GRC residents.
• Must remain awake and alert while on duty.
