Home Health RN Registered Nurse – Empath Home Health

Empath Home Health, a member of Empath Health, is seeking a compassionate full-time Home Health RN to provide skilled nursing care for patients in their homes throughout East Pasco county (Wesley Chapel, Land' O Lakes, Odessa, Dade City, Zephyrhills) & North Hillsborough county (Lutz, New Tampa, Tampa Palms, Temple Terrace, University, and surrounding areas). In this full-time role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence. 

Locations: Home health visits in east Pasco county (Wesley Chapel, Land' O Lakes, Odessa, Dade City, Zephyrhills) and north Hillsborough county including: Lutz, New Tampa, Tampa Palms, Temple Terrace, University)

Schedule: M-F 40 hours. Required weekend/on-call/holiday rotations.

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

Position Requirements

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience.
• Requires prior home health RN experience
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
