Scope of Position: Join our team as a Licensed Mental Health Clinician on the Assertive Community Treatment Team, dedicated to supporting adults diagnosed with serious mental health conditions. This role offers a structured schedule, professional development opportunities, and the chance to make a meaningful impact in the lives of adults with mental health challenges in vibrant NYC communities.
Essential Position Functions:
- Collaborate with a dynamic multi-disciplinary team to deliver comprehensive services aligned with program objectives.
- Participate actively in daily team meetings to strategize and optimize client care plans.
- Provide a substantial number of community-based services monthly, emphasizing client-centered support.
- Ensure timely and thorough documentation to uphold high standards of care and compliance.
- Engage proactively with clients, families, and community partners to empower clients in achieving treatment and recovery milestones.
- Coordinate client care across various settings, including hospitals and substance use treatment facilities, ensuring seamless transitions and holistic support.
- Offer 24-hour crisis intervention on a rotating basis, conducting thorough assessments and developing personalized crisis and relapse prevention plans.
- Utilize evidence-based treatments like cognitive behavioral therapy and motivational interviewing to address mental health and substance abuse needs effectively.
- Support clients in accessing essential resources such as housing, entitlements, employment, education, and legal advocacy, fostering community integration and holistic well-being.
- Implement persistent and strength-based outreach strategies to engage clients actively in their treatment and recovery journey.
Qualifications
- Experience: Minimum two years working with adults diagnosed with serious mental health conditions.
- Education: MSW, LMSW, LMHC, LMHC-LP required.
- Department: ACT/FACT Teams (Assertive Community Treatment/Flexible Assertive Community Treatment).
- Schedule: Monday – Friday, 9:00 AM – 5:00 PM for work-life balance.
- Location: Choose between Manhattan or Bronx for your convenience and accessibility.
- Must have or be eligible to have NY State ID (non-driver or driver's license).
- Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
- Spanish speaking is highly desirable.
- Experience working with homeless and/or precariously housed populations is preferred but not required.
- Knowledge of homeless resources, NYC shelter systems, and MTA transit systems is a plus.
- Knowledge of counseling principles and methods for mental illness and substance use disorders.
- Knowledge of treatment, rehabilitation, and community support programs as they relate to recipients/residents, families, and staff.
- Knowledge of techniques for identifying, assessing, and preventing potentially violent behavior, including crisis management and de-escalation techniques.
- Ability to develop, evaluate, implement, and modify treatment interventions to meet the needs of individual recipients.
- Ability to prepare accurate and timely reports.
