Qualifications
To provide clinical social work services to families and children with intensive need. To provide services with a home-based family centered approach in multiple settings.
MAJOR RESPONSIBILITIES:
DIRECT CLINICAL SERVICES: Provides individual, family, or group counseling services to parents and children in their home, in the office, or in the school. Provides parent education and consultation.
CASE MANAGEMENT / SERVICE COORDINATION: Makes home visits to support families. Facilitates communication between agencies on behalf of the child. Attends team meetings to plan and monitor services. Provides case management, collateral contact, and social support.
CONSULTATION / EDUCATION SERVICES: Provides consultation services to teachers, school personnel and other professionals. Consultation can be educational or evaluative.
ADMINISTRATIVE: Attends all UCS and community meetings necessary to promote coordination and communication. Follows all UCS record keeping procedures and performs other duties as necessary.
*In addition to the above, performs other duties as assigned.
REQUIRED QUALIFICATIONS:
- Master’s Degree in social work or related degree with some experience providing counseling and case management services to families and children.
- Licensure, or on track for licensure, in Vermont as a Social Worker, Psychologist, or Mental Health Professional.
- Valid Driver’s License required
- Typing and navigating, Microsoft Windows literacy, Email usage, EMR
