Summary/Objective
The Cardiac Rehab Nurse is responsible for professional nursing care and related assistance to patients experiencing illnesses related to cardiovascular issues requiring admission to the service. The Cardiac Rehab Nurse supports the medical care of patients requiring Cardiac Rehab services under the direction of the Medical Staff pursuant to the mission and objectives of the Hospital.

- Provides health care teaching to the patient and/or family with documentation of this teaching with a focus on immediate care needs and preparation for transfer to another nursing unit when applicable. Assists patient in recognizing and coping with psychological stresses related to acute illness and possible consequences of the illness on future health status. Facilitates patient self-care when appropriate.
 - Performs patient assessments (including re-assessments) in accordance with standards of care with documentation of these assessments as a basis for planning of each patient's care. Formulates a plan of care for each patient based on nursing diagnosis/patient problems derived from patient assessments. Delegates nursing care activities to member of the nursing team based on patient care needs and competencies of available nursing staff as necessary.
 - Applies principles of growth and development over the life span to identify patient and family requirements relative to age-specific needs. Confers with patients and families to address discharge needs i.e. regimen of home treatment, patient's level of functional independence and other details of required care (training, equipment, referral to other facility or healthcare provider).
 - Manages scheduling of own patient load and other tasks to allow smooth work flow and effective patient care. Including special projects and miscellaneous duties as needed to promote the smooth work flow of the department.
 - Reviews patient's progress with other members of the patient care team. Communicates with physician and Medical staff to inform or consult regarding patient status, progress and recommended treatment changes. Successful outcomes noted in charts or appropriate rationale for unsuccessful outcome documented.
 Ensures implementation of operational standards which promote the maintenance of patient rights including. Organizes patient care and general Cardiac Rehab Department activities to accomplish required functions with delegation of these activities to other staff as appropriate.
Required Education and Experience
Graduate of an accredited nursing program
1 - 3 years related experience
Required License/Certifications
Current RN license in the State of Texas
BLS Certification
ACLS Certification
