Job Description

The Highlands at Brighton, an affiliate of UR Medicine, provides more specialty care services than any other transitional or long term care facility in our region. We are looking for an experienced Social Worker to join our team!

As a Social Worker for our Rehab unit you will be responsible for performing case management duties, counseling, and continuously advocating for our residents.

Responsibilities:

  • Develops and maintains therapeutic relationships with residents and their families or significant others
  • Provides adjustment and bereavement counseling, crisis intervention, and problem solving services
  • Advocates for the best quality of life for residents
  • Acts as a liaison between family/significant others and The Highlands at Brighton
  • Conducts and documents psycho social assessments and interviews
  • Maintains working knowledge of interdisciplinary programs; hospice, rehabilitation, and long term care
  • Coordinates and completes discharge planning of residents

Job Requirements

  • Bachelors degree in Social Work required; Masters degree preferred
  • Previous experience as a Social Worker within an acute care setting required
  • Experience utilizing Electronic Medical Records (EMR) systems preferred
  • Interest and ability to work in a fast-paced environment required
