Position Summary

Responsible for the direct care of older adults in a program setting that supports the interests and needs of individuals, often with disabilities, including intellectual disabilities.

Essential Duties and Responsibilities

  • Ensure the implementation of high-quality individual support plans.
  • Promote and follow Delta’s Risk Management Program for the health and safety of individuals and employees.
  • Engage individuals in meaningful activities at home and community
  • Maintain the daily operations of the home, such as nutritious meal preparation based on protocol diets, cleaning activities in compliance with health, safety, and cleanliness standards.
  • Assist in basic living activities of each individual, including appearance, nutrition, recreation, and socialization.
  • Pursue community integration for each individual
  • Act as a role model to the individuals in areas such as appearance, emotional stability, and social behavior.
  • Encourage and work with individuals to progress towards their outcomes and record data
  • Contribute as a member of the interdisciplinary process in:
  • Assessing the desires, strengths, and needs of individuals in residential services
  • Developing objectives and outcomes
  • Evaluating progress toward independence in outcomes
  • Ensuring proper medication administration
  • Complete and submit in a timely manner, required records and forms such as:
  • Physical Examination Forms
  • Medical Summary
  • Dental Visit Summary
  • Incident/Unusual Incident Report
  • Individual financial ledgers
  • Bi-Weekly petty cash forms and gas ledgers
  • Ensure the proper and timely completion and maintenance of records, including:
  • Communication Logs
  • Daily Progress Notes
  • Data Collection Forms
  • Medication Logs
  • Outcome Data
  • Communicate continuously with your Supervisor
  • Secure the home and its’ contents by following prescribed standards.
  • Maintain the proper staff ratio as per the ISPs of all individuals.
  • Regular attendance is essential
  • Comply with the following safety policies and procedures:
  • Maintain a safe and clean home environment at all times
  • Report unsafe conditions immediately
  • Participate in activities to protect the safety of employees and individuals of Delta
  • Adhere to the use of universal safety precautions and health precautions and health regulations with all individuals
  • Participate in training and education on health and safety issues and techniques yearly in a timely manner
  • Complete fire drills
  • Administer meds as per procedure and report errors
  • Follow on-call policies and procedures
  • Must be an approved driver and must maintain that status.
  • Attend meetings and visit the homes, schools, workplaces, and recreational sites of support recipients.
  • Perform other duties as directed to support service to the individuals, the division and Delta.
  • Perform such other duties that may be assigned for providing excellent customer service and for the most effective operation of the company.
  • Attend monthly staff meetings and all required training programs as assigned by the supervisor.
  • Transport individuals in personal and company vehicles as required.
  • Cooperate with the Department of Human Services (ODP in PA) and DDD (NJ) staff in any investigation or inspection.
  • Cooperate with the company and regulatory agencies personnel during any inspection or investigation.
  • Cooperate with all internal or external audits or investigations as a condition of continued employment.
  • Complete all paperwork accurately, including, but not limited to, Critical Logs, IHP data, etc.
  • Comply with Delta’s policies and DDD licensing standards.

 

When working an overnight shift - (ALL SHIFTS are AWAKE shifts)

  • Ensure that you check each individual in their bedrooms every half hour.
  • Clock into the Vista transfer program every hour.
  • Follow the Duty List attached for your nightly tasks.
  • Each home may have specific duties that will be assigned.
  • Responsible for working with the individuals in the morning.
  • Assist with personal hygiene-toileting, teeth brushing, etc.
  • Assist with ensuring individuals are dressed properly for the weather
  • Preparing them for their day programs as applicable
  • Assist the individuals with making their beds and straightening their rooms.
  • Responsible for preparing appropriate breakfasts.
  • Responsible for preparing appropriate lunches for those who take a lunch to their programs.
  • Secure the home and its’ contents by following prescribed standards.

  • Successful completion of background screenings and clearances:
    1. Criminal History; fingerprinting
    2. NJ - Child Abuse Record Information (CARI)
    3. PA - Childline checks
    4. Drug Testing
    5. NJ - Central Registry
    6. PA - E-Patch Records
    7. Exclusion Screening
    8. Physical/TB screening
  • Ability to read, write, and speak English; perform arithmetical calculations
  • Ability to use excellent oral and written communication skills.
  • Ability to use telephone, voice mail, copier, facsimile machine, computer, and calculator.
  • Ability to work effectively with people, facilitating their growth and development.
  • Ability to use acquired knowledge of supervisory and administrative principles and methods.
  • Ability to operate vehicles with a valid driver’s license in accordance with applicable state laws, insurance provider requirements, and company policies and procedures.
  • Ability to assess information from plan information to facilitate interactions.
  • The ability to frequently exert up to fifty (50) pounds of force (lifting).

Position Qualifications

A high school diploma or equivalent (GED). Exceptional interpersonal communication skills. Excellent verbal and written communication skills. Ability to work on assigned tasks as well as to accept direction on a given assignment. Ability to resolve non-routine situations properly. Ability to manage multiple and sometimes conflicting priorities.
