Homestead Village, is a thriving non-profit, CARF-CCAC accredited, Life Plan Community located just outside vibrant Downtown Lancaster. We are seeking a highly skilled, enthusiastic, & passionate Licensed Practical Nurse (LPN) to join our compassionate community as we enhance independence and dignity for all our Residents.
Position/Schedule: Part Time, non-exempt; 6:45PM - 7:15AM; 2 shifts per week - rotating 3 week schedule
- Week 1: Wednesday & Thursday
 - Week 2: Monday & Tuesday
 - Week 3: Friday & Saturday
 
Location:
- Main Apartment Building; Supportive Servies
 
Base Pay: Determined based upon years licensed and experience. LPN's with less than 1 year experience start at $25.00; Plus applicable shift & weekend differentials
Shift/Weekend Differentials:
- $1.50 additional from 3:00pm-11:00pm (Mon-Fri)
 - $0.75 additional from 11:00pm-7:00am (Mon-Fri)
 - $1.25 additional on weekends (Saturday & Sunday)
 
Qualifications:
- Currently licensed in the Commonwealth of PA as a Licensed Practical Nurse
 - Minimum of three years as an LPN
 - Minimum of three years of supervisory experience
 - Experienced in developing support plans for elders
 - Familiarity with Personal Care Home Regulations
 - Hands-on experience in ADL assistance
 - CPR/AED certified or able to be certified
 - Written & verbal skills to communicate with client, families, and staff.
 - Able to perform the essential job functions of the job description.
 
Responsibilities (the following is an overview of the responsibilities of the position and should not be considered all inclusive):
- Establish respectful, warm relationship with residents, perform rounds
 - Admission of residents
 - Develop/update of resident pre-screen, assessment, and support plans
 - Communicate, as necessary, with physicians and resident families.
 - Staff supervision and coaching
 - Assist with meeting resident activities of daily living as needed/indicated.
 - Assist with medication administration as needed
 - Participate in weekly PCH Supervisory meetings
 - Assist with emergency response campus-wide, as needed
 - Attend all required staff meetings and complete monthly education on Relias by dates due.
 - Adhere to all agency policies and procedures, federal, state, and local regulations, and OSHA safety standards.
 - Perform other duties as assigned
