The Prince George’s County Health Department is currently seeking qualified applicants to fill a Community Health Nurse, (Community Health Nurse I/II) grade A18-A21 position, in the Environmental Health/Disease Control Division.
About the Position
This is an entry level and full performance professional nursing position, whereby the incumbent conducts communicable and vector borne disease investigations, provides nursing services, direct patient care, disease education, and counseling/coordination services. The incumbent must have and maintain a valid Registered Nurse License Issued by the Maryland Board of Nursing or a license recognized by the Compact State Agreement. The incumbent will be the point of contact for the Perinatal Hepatitis B program and is supervised and evaluated by a higher-level Community Health Nurse in accordance with departmental rules, policies, and procedures. Perform Rabies exposures investigations under the supervision of the Rabies coordinator, a supervisor and the program chief.

This plan emphasizes improving core functions and operations, including protecting the public from health threats, implementing strategies to support healthy living, and maintaining a focus on vulnerable populations, while developing efficient internal processes to support these activities.Position Duties
- Assesses, develops, and implements nursing care plans to evaluate patients.
- Provides nursing care services regarding communicable diseases.
- Documents and maintains patients' medical records in accordance with the health department and Division's Protocols.
- Triage and assess patients and contacts health needs.
- Communicates health promotion and prevention concepts to clients.
- Educates and refers patients to patient care and counseling services.
- Conducts interview and provides appropriate medication, education, and case management to reduce the transmission of diseases.
- Assesses clients' condition and need for mediation and vaccination.
- Assists with collecting, evaluation, and interpreting epidemiological, and statistical data pertaining to investigations and surveillance activities.
- Prepares investigation/summary reports.
- Collects, receives and prepares infectious disease specimens for transport to the State lab.
- Uses appropriate PPE during the performance of clinical duties.
- Interprets lab data and appropriately notifies patients and/or providers of results.
- Records case data investigation activities into the National Electronic Disease Surveillance System (NEDSS) and performs clinical documentation in the Appropriate Electronic Health Record.
- Performs other tasks and duties assigned, which may not be specifically listed in the position's description; however, are within the general occupational category and responsibility level typically associated with the employee's class of work.

- Graduation from an accredited school, college, university or an equivalent program with a bachelor's degree in nursing or a related health field or an equivalent combination of education and experience.
- Possession of a valid license to practice as a Registered Nurse in the State of Maryland issued by the Maryland State Board of Nursing at the time of appointment.
- Possession of a valid license to operate a motor vehicle when required for the performance of job-related duties at time of appointment.
- Possession of a valid cardiopulmonary resuscitation (CPR) certification.
