Role Summary:
The Street Medicine Physician (MD/DO)/HIV Specialist is a key clinical position for the Street Medicine Team at San Francisco Community Health Center that seeks to provide quality care for the unhoused unsheltered community, HIV positive individuals, LGBT community, people of color, and other underserved communities in San Francisco and beyond.
The MD/DO is an independent, licensed, advanced provider who provides primary and HIV medical, mental health and MAT evaluation and care to patients in a variety of settings including on the street, at encampments, in temporary and permanent housing and in a clinical setting. This activity is performed in collaboration with other members of the interdisciplinary street medicine team. Additionally, the provider will partner with SFCHC HIV services programs to offer on and off-site care to patients within their intensive case management services. Outside of direct patient care, the incumbent may occasionally be responsible for the supervision of medical residents, interns and NP students if appropriate per licensure. In addition, the incumbent will be asked to lead quality initiatives such as clinical protocol development.
Essential Duties:
Clinical Practice:
- Practices under the Medical Board of the State of California.
- Complies with State Laws on Practicing Medicine.
- Independently assesses and treats acute and chronic clinical problems common in both episodic and primary care settings, as well as primary care specialty areas including but not limited to HIV, hepatitis C, sexual health, reproductive health / family planning, transgender health and urgent care issues that arise.
- Independently assesses and treats common psychiatric conditions including mood disorders, psychotic disorders, trauma related disorders, and substance use disorders in collaboration with the team’s behavioral health providers.
- Performs and documents physical and mental health assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained.
- Provides appropriate counseling and education to patients, their families and significant others.
- Plans patient care based on in-depth knowledge of the specific patient population and/or protocol, anticipating and identifying medical and/or psychological problems commonly encountered including the consideration of the patient’s cultural background, level of understanding, personality and support systems.
- Orders and interprets data obtained from diagnostic tests and laboratory studies, ordering the appropriate medications and treatments and providing patient and staff education.
- Serves as patients’ advocate.
- Refers patients for consultation when indicated i.e. emergency room, dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, mental health services, social service, and dietary.
- Provides full primary care in a variety of settings including on the street, at encampments, in temporary and permanent housing and in a clinical setting.
- Participates in patient care rounds and conferences. Communicates patient management strategies to members of the patient care team. Collaborates with members of the interprofessional team to ensure that patient management strategies are successful in meeting patient care needs.
- Supervises nursing, medical residents, interns and NP students to meet objectives of clinical placements.
- In coordination with the Street Medicine leadership team, creates protocols and standing orders for clinical practice.
- Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, (HIPAA, OSHA, OIG guidelines, other State and Federal laws).
Administrative Duties:
- Participates in continuous quality improvement measures included evaluation of clinical treatment and medical record evaluations.
- Works collaboratively in the submission of accurate and timely documentation and billing.
- Implements, monitors, and reports on quality management activities in accordance with regulatory guidelines and agency policies.
Qualifications:
- Must hold a diploma from a school accredited by the Liaison Committee on Medical Education (LCME) OR an osteopathic medical school by the American Osteopathic Association (AOA).
- MD/DO must have completed not less than 12 months of graduate medical education or its equivalent OR holds certification from the Educational Commission for Foreign Medical Graduates (ECFMG).
- MD/DO has successfully completed not less than 12 months of graduate medical education through the first-year level of graduate medical education or its equivalent Board Certified in Primary Care (Family Practice/Internal Medicine/Med Peds).
- HIV specialist certification via AAHIVM or comparable training (i.e. HIV Fellowship completion).
- Current provider card in Basic Life Support (BLS) and/or Advanced Cardiac Life Support (ACLS).
- DEA license and comfort prescribing buprenorphine.
- Strong communication and problem-solving skills.
- Experience working in a community health center.
- Experience working in street medicine preferred.
- Sensitivity to HIV issues, as well as gender, gender identity, sexual orientation, ethnicity, and language diversity.
- Experience and knowledge of working with underserved and marginalized populations, especially LGBTQ communities and communities of color.
- Strong knowledge base of HIPAA and OSHA.
- Strong computer skills, including Word, Excel, and PowerPoint and familiarity with electronic medical records.
- Experience and ability to work independently.
- Experience and ability to influence and collaborate in a diverse and cross-functional workplace.
- Experience and ability to identify needs and concerns and provide proactive solutions.
- Up-to-date with vaccinations, especially the latest COVID-19 and flu vaccines.
- Proof of TB clearance (issued within the past 12 months).
- Must be able to sit, stand, kneel and walk for extended periods of time.
