GENERAL PURPOSE:
Leads by example and leads others to promote successful aging by understanding the needs and preferences of residents. Leads others to develop a well-trained, compassionate Wellness team to promote dignity and choice. Leads to achieve positive business and performance results. Ensures a productive, safe, and professional environment for all community team members. Promotes a positive community image that supports a fully occupied community.
PRIMARY RESPONSIBILITIES:
- All responsibilities are to be performed within scope of practice per state or provincial regulations.
- Maintains staffing schedules in accordance with current occupancy of the community including
- creating two-week schedules for Charge Nurses and wellness team members
- Maintains awareness of staff schedules to minimize unapproved overtime
- Coordinates coverage for staff call offs to maintain optimum staffing ratios
- Ensures staff have been assigned appropriately through Eldermark, ADP and the Relias learning system
- Ensures staff have been assigned iPads and are logged in during their shift and ensures that all equipment is returned at end of shift
- Ensures staff have been assigned community radios and that all equipment is returned at end of shift
- Assists with adjusting care groups in all applicable systems as residents move in and out of the community
- Double checks that all new resident move ins are assigned an appropriate wellness band
- Assists in monitoring of resident bands to ensure they remain fully charged and operational
- Assists with establishing records for new residents and facilitating the transition of new resident into the YourLife community as needed.
- Assists with new RSA and other wellness team members orientation and training on floor
- Assists with monitoring of supply program • Alerts families when resident supplies are needed
- Manages resident shower schedule and ensures new residents are added
- Assists with tracking of CPR certifications of staff and when classes need to be scheduled
SECONDARY DUTIES:
- Serves as a backup to fulfill a shift as needed when team members call off their shift as requested
- Measures weight and vital signs.
- Collects, stores, and ensures transport of certain specimens for laboratory studies.
- Ensures physician orders, laboratory reports, and other data is scanned and filed in resident chart.
- Adheres to emergency protocols, including performing CPR and administering first aid as necessary.
- Promotes and adhere to service plans, informing DOW of suggestions to improve current resident
- needs and preferences as well as successful service approaches.
- Reads Communication Log at beginning of each shift; enter pertinent information in Communication Log by end of each shift.
- Reads Shift Change Notes at beginning of each shift; enter pertinent information in Shift Change Notes by end of each shift.
- Performs other duties as assigned
SKILLS and QUALIFICATIONS:
- Must be at least 21 years old.
- Demonstrated ability to communicate effectively in English, both verbally and in writing.
- Meet state or provincial health related requirements.
- Maintain any other certifications as required by state or provincial regulations.
- Must be computer literate and able to use Word and Excel programs
PHYSICAL REQUIREMENTS:
Physical Demands include: Standing, Walking, Sitting, Bending, Reaching, Pushing, Pulling, Climbing Stairs, Grasping, Manual Dexterity, Stooping, Kneeling, Crouching, Vision Acuity, Talking, and Hearing. The weight determined to be appropriate for a Wellness Coordinator to lift with or without reasonable accommodations is up to 50 lbs.
