POSITION SUMMARY

Oversees all self-management and community health programs for the population health and community engagement focus of Sky Lakes. Oversee, direct, and report accurate data collection and evaluation for the programs. Supervise internal closed-loop referral process and assist community partners to establish a system using electronic health record software. Lead projects surrounding reimbursement strategies for sustainable self-management programs. Ongoing interfacing with community partners and coalition members for continued program development. Manage and direct activities of the program coordinator. Responsible for ensuring professional and efficient program delivery and fidelity of all evidencebased programs. Oversees and ensures all grant requirements are met in a timely manner. Oversees budgetary aspects of grant funds.

QUALIFICATIONS

Required: Bachelor’s Degree in health/social service related field. 1-2 years’ experience in management of programs and personnel. Ability to work independently without constant supervision; Ability to meet deadlines; Ability to effectively communicate with clients, medical personnel, community partners and others they may come into contact with, in a pleasant and constructive manner; Strong communication skills including written and verbal; Ability to prioritize workload in a timely manner; Ability to take direction from lead, manager or director regarding work activities and procedures.

Preferred: Health/social service experience; experience working in multi-cultural settings, bilingual.

TYPICAL PHYSICAL/MENTAL DEMANDS

Must have good general health, be able to frequently stoop, bend, and reach. Sit long periods, occasional walking and standing. Able to lift 50 pounds or more. Must demonstrate calmness and ability to prioritize tasks under busy situations. Must possess the visual, auditory and fine motor skills, required for administrative and clerical functions. Must have reliable transportation and a valid driver’s license.

ESSENTIAL JOB FUNCTIONS

  • Supervises program coordinator.
  • Manage self-management workshops and leader trainings.
  • Vet, evaluate, and mentor workshop leaders.
  • Coordinate Master Training and work on professional development of Master Trainers.
  • Works closely with clinic staff, medical providers, and community partners to ensure efficient and successful delivery of programs.
  • Oversee management of program referrals.
  • Evaluates program efforts and system impact.
  • Facilitate monthly meetings, trainings, and outreach events.
  • Potentially work nights and weekends for outreach events.
  • Ensure data collection is accurate, complete, and submitted to appropriate agencies.
  • Direct recruitment efforts to obtain program participants.
  • Ensure availability of appropriate program equipment and materials.
  • Oversee development and dissemination of program marketing materials.
  • Strategic planning for sustainability of programs.
  • Identify and develop new community partnerships both locally and statewide.
  • Works closely with clinic staff, medical providers, and community members for delivery of PFAC.
  • Oversee that all grant requirements are completed and submitted as specified.
  • Seek additional grant funds to support programming development and delivery.
